妊娠合并宫颈癌的诊疗进展

Diagnosis and treatment principles of cervical cancer in pregnancy

近年来由于高龄生育妇女比例增加,致使妊娠期合并妇科恶性肿瘤的发病率有所上升。其中最为常见的妊娠期妇科恶性肿瘤为宫颈癌,占71.6%;其次是卵巢恶性肿瘤,占7.0%。妊娠合并宫颈癌的发病率较低,且症状易与妊娠期疾病相混淆,妇科特殊检查也因妊娠而有所限制,因此误诊或漏诊率较高。妊娠期宫颈癌的治疗与患者肿瘤大小、病理类型、妊娠时间、淋巴结受累情况及患者维持妊娠的意愿等多种因素有关,故目前难以确定最佳治疗方案。为了在有效治疗肿瘤和保护胎儿健康之间取得平衡,避免延误治疗和早产,现就妊娠合并宫颈癌的诊治进展进行综述。

In recent years, the incidence of gynecological malignancy during pregnancy has increased due to advanced age. Among them, the most common gynecological malignant tumor in pregnancy is cervical cancer, followed by ovarian cancer. Cervical cancer accounts for 71.6%, and ovarian malignant tumor accounts for 7.0%. The incidence of pregnancy complicated with cervical cancer is low, and symptoms are easy to be confused with pregnancy diseases. Special gynecological examination is also limited due to pregnancy, so the rate of misdiagnosis or missed diagnosis is high. The treatment of cervical cancer in pregnancy is related to the tumor size, pathological type, duration of pregnancy, involvement of lymph nodes and patients willingness to maintain pregnancy. Therefore, it is difficult to determine the best treatment. In order to strike a balance between the most effective treatment of tumor in patients and the protection of fetal health, and to avoid delayed treatment and premature delivery, this article reviews the research progress on the diagnosis and treatment principles of cervical cancer in pregnancy.
