探讨羊水粪染对母儿妊娠结局的影响以及相关性 被引量:1

摘要 目的探讨羊水粪染对母儿妊娠结局的影响以及相关性。方法150例羊水粪染患者,粪染Ⅰ度、Ⅱ度、Ⅲ度各50例,分别作为Ⅰ度组、Ⅱ度组、Ⅲ度组。比较三组患者的妊娠结局,患儿的感染率以及并发症发生率,分析羊水粪染与妊娠结局的相关性。结果Ⅰ度组妊娠结局正常48例、所占比例为96.00%,Ⅱ度组妊娠结局正常42例、所占比例为84.00%,Ⅲ度组妊娠结局正常33例、所占比例为66.00%。Ⅲ度组的妊娠结局正常率明显低于Ⅰ度组和Ⅱ度组,Ⅱ度组妊娠结局正常率低于Ⅰ度组,差异均具有统计学意义(P<0.05)。Ⅲ度组患儿的感染率为28.00%,明显高于Ⅰ度组的2.00%和Ⅱ度组的12.00%,Ⅱ度组患儿的感染率高于Ⅰ度组,差异均具有统计学意义(P<0.05)。Ⅲ度组患儿的并发症发生率为52.00%,明显高于Ⅰ度组的12.00%和Ⅱ度组的28.00%,Ⅱ度组患儿的并发症发生率高于Ⅰ度组,差异均具有统计学意义(P<0.05)。羊水粪染与妊娠结局呈正相关性(r=0.512, P<0.05)。结论羊水粪染对母儿妊娠结局具有一定的负面影响,且粪染程度与妊娠结局情况呈正相关,值得临床注意。 Objective To discuss the effect of meconium stained amniotic fluid on maternal and fetal pregnancy outcomes and its correlation. Methods A total of 150 patients with meconium stained amniotic fluid were divided by meconium stained degree into grade Ⅰ group, grade Ⅱ group and grade Ⅲ group, with 50 cases in each group. Pregnancy outcomes, infection rate and incidence of complications were compared among the three groups, and the correlation between meconium stained amniotic fluid and pregnancy outcomes was analyzed. Results Grade Ⅰ group had 48 normal pregnancy outcomes, accounting for 96.00%, grade Ⅱ group had 42 normal pregnancy outcomes, accounting for 84.00%, and grade Ⅲ group had 33 normal pregnancy outcomes, accounting for 66.00%. Grade Ⅲ group had obviously lower normal pregnancy outcomes rate than grade Ⅰ group and grade Ⅱ group, and grade Ⅱ group had lower normal pregnancy outcomes rate than grade Ⅰ group. Their difference was statistically significant(P<0.05). Grade Ⅲ group had obviously higher infection rate as 28.00% than 2.00% in grade Ⅰ group and 12.00% in grade Ⅱ group, and grade Ⅱ group had higher infection rate than grade Ⅰ group. Their difference was statistically significant(P<0.05). Grade Ⅲ group had obviously higher incidence of complications as 52.00% than 12.00% in grade Ⅰ group and 28.00% in grade Ⅱ group, and grade Ⅱ group had higher incidence of complications than grade Ⅰ group. Their difference was statistically significant(P<0.05). There was a positive correlation between meconium stained amniotic fluid and pregnancy outcome(r=0.512, P<0.05). Conclusion Meconium stained amniotic fluid has a certain negative impact on maternal and fetal pregnancy outcomes, and the degree of fecal staining is positively correlated with pregnancy outcomes. It deserves clinical attention.
