心内科专科护理分级模式的实践与探索

Practice and Exploration of Nursing Grading Mode in Cardiology

目的探讨心内科专科护理分级模式应用效果。方法将2018年3月-2018年9月普通病房收入或者转入的患者按照随机数表法分为对照组和观察组,对照组按照以往临床护理工作模式,由医生下达护理等级医嘱,护士根据医嘱进行护理。观察组采用心内科专科护理分级模式,主要是运用自制的心血管专科疾病病情评估表以及改良Barthel指数评分表对患者进行综合评估,医护合作共同确定护理等级。比较2组死亡率、抢救成功率、护理分级准确率、不良事件发生率、平均住院时间及患者满意度是否存在差异。结果观察组死亡率、不良事件发生率低于对照组(P <0.05);观察组抢救成功率、护理分级准确率及满意度均高于对照组(P <0.05);观察组平均住院时间少于对照组(P <0.05)。结论心内科实施专科护理分级模式可降低患者死亡率、不良事件发生率,缩短平均住院时间,提高患者抢救成功率、护理分级准确率,提升患者满意度。

Objective To explore the application effect of the nursing grade classification model in cardiology department. Methods Patients with general ward income or transferred from March 2018 to September 2018 were divided into control group and observation group according to the random number table method. In the control group, according to the routine clinical nursing work mode, the doctor gave the nursing level, and the nurse took care according to the doctor’s advice. The observation group adopted the cardiology professional nursing grading mode, mainly using the self-made cardiovascular disease assessment form and the improved Barthel index score table to comprehensively evaluate the patients, and the medical cooperation to determine the level of care. The differences between the two groups of mortality, rescue success rate, nursing grading accuracy, adverse event rate, average hospitalization time, and patient satisfaction were compared. Results The incidence of mortality and adverse events in the observation group was lower than that in the control group(P < 0.05). The success rate of rescue, the accuracy of nursing grading and the satisfaction rate of the observation group were higher than those of the control group(P < 0.05). Conclusion The implementation of specialist nursing grading mode in cardiology can reduce the mortality rate of patients and the incidence of adverse events, shorten the average length of hospital stay, improve the success rate of patients, the accuracy of nursing grading and patient satisfaction.
