胃癌患者外周血记忆性T细胞的临床意义

Clinical significance of peripheral blood memory T cells in patients with gastric cancer

[目的]研究胃癌患者外周血记忆性T细胞(TM)的分布特征,分析TM与临床病理特征的关系,探讨其在胃癌治疗中的临床应用价值。[方法]选取2018年7月^2018年12月在我院住院的胃癌患者32例为病例组,同期健康志愿者23例作为对照组。应用流式细胞术检测胃癌患者外周血TM的分布比例,并比较其与对照组的差异,分析TM与临床病理特征的关系。[结果]病例组患者外周血CD8^+T细胞比例明显低于健康对照组,CD8^+中枢性记忆性T细胞(TCM)比例、CD8^+效应性记忆性T细胞(TEM)/CD8^+T细胞值、CD8^+TCM/CD8^+T细胞值均明显高于健康对照组,差异均有统计学意义(P<0.05)。CD4^+T细胞比例与肿瘤TNM分期呈负相关,CD4^+TCM比例与组织分化程度呈正相关,CD4^+TEM比例与肿瘤TNM分期、淋巴结转移、浸润深度呈负相关。[结论]胃癌患者外周血TM可用于评价患者的免疫状态,间接判断患者预后;且记忆性T细胞亚群在胃癌的过继细胞免疫治疗中可能有重要的临床应用价值。

[Objective]To study the distribution characteristics of peripheral blood memory T cells(TM)in patients with gastric cancer,we analyze the relationship between TM and clinicopathological characteristics,and explore its clinical application value in the treatment of gastric cancer.[Methods]32 cases of gastric cancer patients hospitalized in our hospital from July 2018 to December 2018 were selected as case groups.23 healthy volunteers during the same period served as the control group.The proportion of memory T cells was detected by flow cytometry,in peripheral blood of patients with gastric cancer.The difference between them was compared with the control group.We analyzed the relationship between memory T cells and clinicopathological features.[Results]The CD8^+T cells in peripheral blood of the patients in the case group were significantly lower than those in the healthy control group,while the CD8^+central memory T cells(TCM),CD8^+effector memory T cells(TEM)and CD8^+TCM/CD8^+T cells were significantly higher than those in the healthy control group(P<0.05).The proportion of CD4^+T cells was negatively correlated with tumor TNM stage,the proportion of CD4^+TCM was positively correlated with tissue differentiation degree,and the proportion of CD4^+TEM was negatively correlated with tumor TNM stage,lymph node metastasis and invasion depth.[Conclusion]Peripheral blood TM can be used to evaluate the immune status of patients with gastric cancer and indirectly determine the prognosis of patients.Moreover,memory T cell subsets may have important clinical application value in adoptive cell immunotherapy of gastric cancer.
