摘要
目的探讨血液灌流联合连续性静脉-静脉血液滤过治疗在严重脓毒症患者中的临床效果。方法将2016年1月—2018年9月就诊于本院的90例严重脓毒症患者随机分为行常规治疗的对照组与行血液灌流联合连续性静脉-静脉血液滤过治疗的观察组,对比分析两组患者的治疗效果。结果血液灌流联合连续性静脉-静脉血液滤过组患者Hb、WBC、BUN、SCr、LAC、hcCRP以及尿量等各项指标治疗后较治疗前均显著改善(P<0.05),且改善情况显著优于对照组(P<0.05);在TNF-α、IL-6及IL-8等炎性因子改善方面,血液灌流联合连续性静脉-静脉血液滤过组患者也显著优于对照组(P<0.05);在重症医学科住院时间、总住院时间及患者死亡率方面,血液灌流联合连续性静脉-静脉血液滤过组均显著优于对照组(P<0.05)。结论对严重脓毒症患者进行血液灌流联合连续性静脉-静脉血液滤过治疗,能够有效改善患者的临床症状,提高其临床治疗效果及免疫功能,利于患者的早日康复,值得广泛应用于临床。
Objective To explore the clinical effect of hemoperfusion(HP) combined with continuous venous venous hemofiltration(CVVH)treating severe sepsis.Methods:90 patients with severe sepsis in our hospital from January 2016 to September 2018 were randomly divided into the control group treated with routine therapy and the observation group treated with hemoperfusion combined with continuous veno-venous hemofiltration.The therapeutic effects of the two groups were compared and analyzed.Results:After treatment,the indexes of Hb,WBC,BUN,SCr,LAC,hcCRP and urine volume were significantly improved in patients with hemoperfusion combined with continuous veno-venous hemofiltration(P<0.05),and the improvement was significantly better than that of the control group(P <0.05).In the improvement of inflammatory factors such as TNF-a,IL-6 and IL-8,hemoperfusion combined with continuous veno-venous hemofiltration group was significantly improved(P<0.05).It was also significantly better than that of the control group(P<0.05).In terms of hospitalization time,total hospitalization time and mortality of patients in the critical medicine department,the combination of hemoperfusion and continuous veno-venous hemofiltration group was significantly better than that of the control group(P<0.05).Conclusion:Hemoperfusion combined with continuous veno-venous hemofiltration for severe sepsis patients can effectively improve the clinical symptoms,improve the clinical efficacy and immune function,and facilitate the early recovery of patients.It is worth being widely used in clinical practice.
