参附注射液联合低分子肝素对小儿脓毒血症血浆NT-ProBNP、Cys、凝血功能和预后的影响

Effects of Shenfu injection combined with low molecular weight heparin on plasma NT-ProBNP, Cys,coagulation function and prognosis in children with sepsis

目的探讨参附注射液联合低分子肝素治疗小儿脓毒血症的临床效果。方法选择2012年12月-2017年12月在河南省郑州市第十六医院儿科收治的80例小儿脓毒血症患儿为研究对象,将患儿以随机数字表法分为A组(n=40)和B组(n=40),A组患儿作为观察组采用参附注射液联合低分子肝素作为治疗方案,B组患儿采用低分子肝素治疗作为对照组,对比分析两组患儿凝血指标、PT-ProBNP、Cys及安全性。结果治疗后A组患儿PT、ATPP低于B组患儿,FIB高于B组患儿,差异有统计学意义(P<0.05);A组患儿治疗后PT-ProBNP、Cys低于B组患儿,差异有统计学意义(P<0.05);A组患儿治疗后SOFA、APACHE评分分别为(4.5±2.1)分、(13.5±5.1)分,B组患儿治疗后SOFA、APACHE评分分别为(6.7±2.6)分、(17.3±4.4)分,差异有统计学意义(P<0.05);A组患儿治疗后DIC、MODS发生率低于B组患儿,差异有统计学意义(P<0.05),两组患儿死亡率比较差异无统计学意义(P>0.05)。结论小儿脓毒血症患儿采用参附注射液联合低分子肝素可以有效降低PT-ProBNP、Cys水平,改善凝血功能,具有较高安全性。

Objective To investigate the clinical effect of Shenfu injection combined with low molecular weight heparin in the treatment of children with sepsis.Methods Eighty children with sepsis admitted to the 16 th Hospital of Zhengzhou City,Henan Province from December 2012 to December 2017 were enrolled.The children were divided into the group A by random number table method.=40)and the group B(n=40),children in the group A were treated with Shenfu injection combined with low molecular weight heparin as the treatment group,and children in the group B were treated with low molecular weight heparin as the control group.Coagulation index,PTProBNP,Cys and safety were compared and analyzed.Results After treatment,the PT and ATPP of the group A were lower than that of the group B.The difference between FIB and the group B was statistically significant(P<0.05).After treatment,group PT-ProBNP and Cys were lower than that of the group B.The difference was statistically significant(P<0.05).The scores of SOFA and APACHE in the group A were(4.5±2.1) points and(13.5±5.1) points respectively.The children in the group B were treated with SOFA and APACHE.The scores were(6.7±2.6) and(17.3±4.4),respectively,and the difference was statistically significant(P<0.05).The incidence of DIC and MODS in the group A was lower than that of the group B.The difference was statistically significant(P<0.05).There was no significant difference in mortality between the two groups(P>0.05).Conclusion Children with sepsis in children with Shenfu injection combined with low molecular weight heparin can effectively reduce the levels of PT-ProBNP and Cys,improve coagulation function,and have higher safety.
