Liu Fu-xing’s Clinical Experience of Treating Solar Dermatitis Based on Syndrome Differentiation

刘复兴教授认为禀赋不耐是日晒疮发病的基础,光热毒邪为发病的诱因,湿、热、毒邪为致病因素,应以清热除湿、凉血解毒为治疗大法。临证时结合地域特点给予自拟中药协定处方分型论治:湿热蕴毒型给予化裁龙胆泻肝汤加减;血热风燥型给予自拟荆芩汤加减;热毒炽盛型予以黄连解毒汤加减。刘复兴教授在诊治日晒疮中善于运用土茯苓、青蒿、昆明山海棠等特色药物,同时强调皮肤病的治疗除辨证准,用药精之外,忌口也是提高疗效的重要环节,如光敏性皮肤病忌食芥蓝菜、灰菜、油菜等。

Professor Liu Fu-xing thinks that weak natural endowment is the basic cause of solar dermatitis;excessive light and heat are the inducement of the disease;dampness-heat and toxin are the pathogenic factors. As a result,the treatment principle focuses on removing heat and dampness,cooling blood and detoxifying. In clinical practice,the regional characteristics should be combined to make TCM agreement prescription for classification and treatment. For syndrome of dampness-heat-toxin accumulation, Modified Gentian Liver-Draining Decoction can be used. For blood-heat and wind-dryness syndrome,Self-Made Modified Jingqin Decoction ( JQD) can be used. For excessive heat-toxin syndrome,Modified Coptis Toxin-Resolving Decoction can be used. Professor Liu is good at using characteristic drugs,such as Tufuling ( Rhizoma Smilacis Glabrae),Qinghao ( Herba Artemisiae Annuae),Shanhaitang( Begonia yunnanensis) from Kunming. At the same time,besides emphasizing exact syndrome differentiation and proper drug-use,he believes that dietetic restraint is also an important link to improve the curative effect,for example, Patients with photosensitive dermatosis should avoid Chinese kale,gray rape,rape and so on.
