Zheng Bangben’s Experience of Treating Chronic Liver Disease

慢性肝病大多由湿热、血瘀、肝郁、气滞等多种病因所致。郑邦本教授在慢性肝病的治疗中以治病求因,扶正祛邪、病证结合、攻补兼施,肝脾同治等为主要学术思想,采用清热利湿、疏肝利胆、疏肝解郁、理气健脾、活血化瘀、软坚散结等综合疗法,将治疗肝病的中药主要分为降转氨酶、降黄疸、抗纤维化、抗病毒、调节免疫等5大类。如肝郁症状明显者选用逍遥散、柴胡疏肝散、四逆散等;脾功能亢进导致全血细胞减少出现血虚证候明显者选用四物汤,或单药阿胶、鸡血藤等,或采用自拟经验方升血方;脾虚者选用四君子汤,或六君子汤,或香砂六君子汤;兼痰湿者选用二陈汤,或厚朴、莱菔子、苍术等化湿除痰之品。

Chronic liver diseases are mostly caused by damp-heat,blood stasis,liver depression and qi stagnation. Professor Zheng Bangben's main academic thought in the treatment of chronic liver diseases is to seek the cause of the disease,to strengthen healthy qi and dispel pathogens,to combine disease with syndrome,to attack and supplement simultaneously,and to treat both liver and spleen simultaneously. He uses comprehensive therapies such as clearing away heat and dampness,soothing liver and promoting bile secretion and relieving depression,regulating qi and invigorating spleen,activating blood circulation and removing blood stasis,and softening hardness and dissipatingmass. The traditional Chinese medicines for treating liver diseases are divided into the following five categories: hypotransaminase,hypojaundice,anti-fibrosis,anti-virus and immune regulation. For obvious symptoms of liver depression,Free Wanderer Powder,Bupleurum Liver-Soothing Powder,Frigid Extremities Powder are often used. For obvious syndrome of blood deficiency due to hypersplenism leading to panhematopenia,Four Substances Decoction or single drug,such as Ejiao ( Colla Corii Asini),Jixuetong ( Caulis Spatholobi),or self-made empirical prescription Blood- Generating Decoction is often used. For spleen deficiency,Four Gentlemen Decoction,or Six Gentlemen Decoction,or Costusroot and Amomum Six Gentlemen Decoction is often used. For those combined with phlegm and dampness,Two Matured Substances Decoction or Houpo ( Cortex Magnoliae Officinalis),Laifuzi ( Semen Raphani) and Cangzhu ( Rhizoma Atractylodis) are often used to remove dampness and phlegm.
