丹芪益肾方治疗气阴两虚夹瘀型糖尿病肾病

Danqi Yishen Decoction in Treatment of Diabetic Nephropathy with Qi and Yin Deficiency as Well as Blood Stasis Syndrome

目的:观察丹芪益肾方治疗气阴两虚夹瘀型糖尿病肾病的临床疗效及对胰岛素样生长因子(insulin-like growth factor,IGF)的影响。方法:110例糖尿病肾病患者随机分为观察组55例和对照组55例。对照组患者给予还原型谷胱甘肽注射液,观察组患者在对照组治疗基础上给予丹芪益肾方。比较两组患者临床疗效,中医证候积分,血糖、血脂水平,胰岛功能,肾功能及IGF水平。结果:观察组有效率(92.73%)显著高于对照组(80.00%)(P<0.05)。与治疗前比较,治疗后两组患者中医证候积分显著降低(P<0.05);观察组中医证候积分显著低于对照组(P<0.05)。与治疗前比较,治疗后两组患者相关血糖、血脂水平显著降低(P<0.05);观察组以上指标均显著低于对照组(P<0.05)。与治疗前比较,两组患者空腹胰岛素、胰岛素抵抗指数均显著降低,β细胞胰岛素分泌功能显著升高;观察组以上指标均改善程度显著优于对照组(P<0.05)。与治疗前比较,治疗后两组患者肾功能显著好转(P<0.05);观察组肾功能显著优于对照组(P<0.05)。与治疗前比较,治疗后两组患者IGF-1、IGF-2显著降低(P<0.05);观察组以上指标均显著低于对照组(P<0.05)。两组患者治疗期间均未出现明显不良反应。结论:丹芪益肾方治疗气阴两虚夹瘀型糖尿病肾病疗效显著,可明显改善患者血糖、血脂、肾功能以及胰岛功能。

Objective:To observe the clinical efficacy of Danqi Yishen Decoction in treatment of diabetic nephropathy with Qi and Yin deficiency Syndrome and its effect on insulin-like growth factor(IGF).Methods:110 patients with diabetic nephropathy were randomly divided into observation group(55 cases)and control group(55 cases).The control group was given reduced glutathione injection,and the observation group was given Danshen Yishen Decoction in addition to that of the control group.The clinical efficacy,TCM syndrome scores,blood glucose,blood lipid levels,islet function,renal function and IGF levels between the two groups were compared.Results:The effective rate of the observation group(92.73%)was significantly higher than that(80.00%)of the control group(P<0.05).Compared with that before treatment,the TCM syndrome scores of the two groups were significantly lowered after treatment(P<0.05);the TCM syndrome scores of the observation group were significantly lowered than that of the control group(P<0.05).Compared with that before treatment,the blood glucose and blood lipid levels of the two groups were significantly lowered after treatment(P<0.05);the above indicators in the observation group were significantly lowered than that of the control group(P<0.05).Compared with that before treatment,the fasting insulin and insulin resistance index of the two groups were significantly decreased,while the insulin secretion function ofβcells was significantly increased.The improvement of the above indexes in the observation group was significantly better than that of the control group(P<0.05).Compared with that before treatment,the renal function of the two groups was significantly improved after treatment(P<0.05);the renal function of the observation group was significantly better than that of the control group(P<0.05).Compared with that before treatment,IGF-1 and IGF-2 were significantly lowered in the two groups after treatment(P<0.05).The above indexes in the observation group were significantly lowered than those in the control group(P<0.05).No significant adverse reactions occurred during the treatment period.Conclusion:Danqi Yishen Decoction is effective in treating Qi and Yin deficiency as well as blood stasis type diabetic nephropathy,which can significantly improve blood sugar,blood lipids,renal function and islet function.
