非特异性疼痛对老年帕金森病患者睡眠质量的影响及其危险因素分析

The Effect of Non-specific Pain on Sleep Quality of Elderly Parkinson's Disease Patients and its Risk Factors

目的观察老年帕金森病(PD)患者发生非特异性疼痛对睡眠质量的影响,并探讨其危险因素。方法收集柳州市人民医院2013-2018年收治的老年PD患者318例,分为疼痛组140例和非疼痛组178例,另外选择同期于柳州市人民医院进行体检的健康老年人150例作为对照组,比较三组患者的匹兹堡睡眠质量指数量表(PSQI)和疲劳量表-14(FS-14)评分,并对318例PD患者出现疼痛的危险因素进行分析。结果三组PSQI比较显示,睡眠持续性和使用睡眠药物条目组间差异无统计学意义(P>0.05);睡眠潜伏期、主观睡眠质量、习惯性睡眠效率、日间功能紊乱、睡眠紊乱和总分差异均有统计学意义(P<0.05);三组FS-14评分差异有统计学意义(P<0.05)。病程、Hoehn-Yahr分级、合并抑郁、合并焦虑是老年帕金森病患者疼痛发生的独立危险因素。结论非特异性疼痛会对老年PD患者的睡眠质量产生明显影响;尤其是病程长、Hoehn-Yahr分级高及合并抑郁、焦虑的患者,更应重视非特异性疼痛的发生,必要时给予综合治疗。

Objective To observe the effect of nonspecific pain on sleep quality in elderly patients with Parkinson’s disease (PD) and explore its risk factors. Methods A total of 318 elderly patients with PD admitted to Liuzhou People’s Hospital from 2013 to 2018 were divided into pain group (140 cases) and non-pain group (178 cases). Another 150 healthy elderly people who underwent physical examination in our hospital during the same period were selected as control group. The Pittsburgh Sleep Quality Index (PSQI) and Fatigue Scale-14 (FS-14) scores of the three groups were compared, and the risk factors of pain in 318 PD patients were analyzed. Results There was no significant differences in PSQI between the three groups (P>0.05). There were significant differences in sleep latency, subjective sleep quality, habitual sleep efficiency, daytime dysfunction, sleep disorder and total score (P<0.05). There was significant difference in FS-14 score among the three groups (P<0.05). Course of disease, Hoehn-Yahr classification, depression and anxiety are independent risk factors for pain occurrence in elderly patients with Parkinson’s disease. Conclusion Non-specific pain can significantly affect the sleep quality for elderly patients with PD, especially in patients with long course of disease, high Hoehn-Yahr grade, depression and anxiety. More attention should be paid to the occurrence of non-specific pain, and comprehensive treatment should be given as necessary.
