活血生肌中药联合远红外线照射对自体动静脉内瘘的影响

Therapeutic effect of traditional Chinese medicine for promoting blood circulation and promoting granulation combined with far-infrared therapy on autologous arteriovenous fistula

目的观察活血生肌中药外敷联合远红外线照射对于自体动静脉内瘘的血流及血脂、肾功能以及内瘘相关并发症的影响。方法将采用自体动静脉内瘘行长期规律性血液透析治疗的慢性肾衰竭患者70例随机平均分为2组,观察组35例患者选择活血生肌中药膏外敷联合远红外线照射的方式干预自体内瘘,对照组35例患者单纯给予远红外线照射,对比2组患者治疗前及治疗3个月后自体动静脉内瘘血流指标、血脂以及肾功能指标变化情况及并发症发生情况。结果2组患者治疗3个月后自体内瘘内径、血流量以及透析血流量均明显高于治疗前(P均<0.05),且观察组治疗后以上指标明显高于对照组(P均<0.05)。观察组患者治疗后TC、LDL水平均明显低于对照组与治疗前(P均<0.05)。观察组患者自体内瘘相关的并发症发生率均明显低于对照组(P均<0.05)。结论活血生肌中药外敷联合远红外线治疗方式能够有效改善自体动静脉内瘘的血流情况,对血脂指标有一定的改善作用,能够有效避免内瘘狭窄、堵塞等并发症的发生,具有较高的临床应用价值。

Objective It is to observe the effects of external application of traditional Chinese medicine for promoting blood circulation and promoting granulation combined with far-infrared therapy on the blood flow of autologous arteriovenous fistula, blood lipids, renal function and correlated complications of fistula. Methods Seventy patients with chronic renal failure who had autologous arteriovenous fistula and underwent long-term regular hemodialysis were randomly divided into two groups. The observation group ( n =35) were treated with external application of traditional Chinese medicine for promoting blood circulation and promoting granulation combined with far-infrared therapy for autologous arteriovenous fistulamuscle, the control group ( n =35) were given far-infrared irradiation alone. The blood flow of autologous arteriovenous fistula, blood lipids, renal function and correlated complications of fistula were compared between the two groups. Complications. Results The internal diameter, blood flow and dialysis blood flow of autologous arteriovenous fistula in the patients of the two groups were significantly higher than those before treatment ( P <0.05), and the indexes above after treatment in the observation group were significantly higher than that in then control group ( P <0.05). After treatment, the levels of TC and LDL of the observation group were significantly lower than those of the control group and before treatment ( P <0.05). The incidence of correlated complications of fistula in the observation group was significantly lower than that in the control group ( P <0.05). Conclusion External application of traditional Chinese medicine for promoting blood circulation and promoting granulation combined with far-infrared therapy can effectively improve the blood flow of autologous arteriovenous fistula, and has a certain effect on the improvement of blood lipid index, which can effectively avoid complications such as stenosis and clogging with good value for application in clinic.
