肝不藏魂型与心不藏神型失眠症的PSQI差异比较

PSQI Comparison of Insomnia between the Two Patterns of Heart Failing to House Shen and Liver Failing to Store Soul

目的:通过对“中医不寐五神分型诊断法”心不藏神型与肝不藏魂型失眠症特征的研究,以充实完善“中医不寐五神分型诊断法”。方法:纳入肝不藏魂型失眠症患者91例,心不藏神型失眠症患者75例,并对纳入的失眠症患者进行PSQI评分和分析。结果:肝不藏魂组与心不藏神组在睡眠质量、睡眠时间,以及睡眠效率和睡眠障碍上存在差异,且均为心不藏神组高于肝不藏魂组;两组在入睡时间及日间功能障碍的比较上无统计学差异。两组在PSQI总分的比较上存在差异(P<0.01)。结论:心不藏神型的PQSI总分高于肝不藏魂型。心不藏神型不寐患者在PSQI评分上的表现特征主要为入睡时间的延长和睡眠时间的减少;肝不藏魂型不寐患者在PSQI评分上的表现特征主要为入睡时间的延长,日间功能障碍和睡眠质量的下降。我们认为通过分析PSQI量表所得出的心不藏神型与肝不藏魂型失眠症特征与中医不寐五神分型诊断法中所定义的特征相符。

Objective: To enrich and complete the diagnosis method of insomnia by TCM five-Shen classification through the study the patterns of Heart failing to house Shen and Liver failing to store soul. Methods: There were 75 cases of Heart failing to house Shen(the Heart group) and 91 cases of Liver failing to store soul(the Liver group). PSQI scores were analyzed in the two groups. Results: There were differences in sleep quality, sleep duration, habitual sleep efficiency and sleep disturbances between the two groups;of which the scores were higher in the Heart group than those in the Liver group. There were no significant differences in sleep latency and daytime dysfunction between the two groups. There were differences in total PSQI scores between the two groups( P <0.01). Conclusion: The total PQSI score of the Heart group was higher than that of the Liver group. The Heart group was mainly characterized by prolonged sleep latency and shortened sleep duration in PSQI scores. The Liver group was mainly characterized by prolonged sleep latency, more daytime dysfunction and decreased sleep quality in PSQI scores. It is regarded that the characteristics of the two patterns obtained by PSQI scale are consistent with those defined in the TCM five-Shen classification of insomnia.
