手术复位颧上颌复合体骨折对称性恢复的定量评估

Quantitative assessment of symmetry recovery in surgical reduction of zygomaticomaxillary complex fractures

目的 介绍一种定量评估颧上颌复合体(ZMC)骨折术后对称性的方法。 方法 选取2012年4月至2016年11月25例在浙江大学医学院附属第一医院手术复位粉碎性ZMC骨折的患者,根据是否采用导航手术分成两组。使用三维CT模型评估手术结果。在所有模型上确定五对标志点,并根据其坐标计算不对称评分,定量分析组间的对称性差异。 结果 所有患者均恢复良好;3对标记点在导航组中观察到比对照组更好的对称性(P<0.05)。 结论 本研究介绍的分析方法能准确客观的评估颧上颌体骨折的手术效果。

Objective To Introduce a method for quantitative evaluation of postoperative symmetry of zygomaticomaxillary complex (ZMC) fracture. Methods The sample comprised 25 patients who underwent surgical reduction of comminuted ZMC fractures. They were divided into two groups according to the use of surgical navigation. Reduction outcomes were evaluated by using three-dimensional computed tomography models. Five pairs of landmarks were identified on all craniofacial models, and asymmetry scores were calculated based on their coordinates. In quantitative analyses, symmetry wascompared between groups. Results All patients recovered uneventfully. Greater symmetry was observed in the navigation group than in the control group for three of the five pairs of landmarks (P<0.05). Conclusions Three-dimensional computed tomography models presented in this study can accurately and objectively evaluate the surgical outcome of zygomaticomaxillary complex (ZMC) fracture.
