上颌窦内提升术同期植骨与未植骨种植术后效果比较

Comparison of the effect of maxillary sinus elevation with simultaneous bone grafting and non-bone grafting

目的比较上颌窦内提升术同期植骨与未植骨种植术后效果。方法选取2014年8月至2017年8月于郑州大学第一附属医院行上颌窦内提升联合上颌第一磨牙种植术且修复后1 a定期复查的48位患者(48颗种植体)为研究对象。术中上颌窦底填入Bio-oss骨粉材料的24颗种植体为植骨组,术中上颌窦底未填入植骨材料的24颗种植体为未植骨组。术后3个月,采用种植体松动度测定仪测量种植体稳定系数(ISQ),分析植骨与否对种植体松动度的影响;根据种植修复后1 a复诊种植体X线根尖片显示骨吸收的实际高度,分析种植体边缘骨吸收情况。结果术后3个月,两组ISQ比较,差异无统计学意义(P>0.05)。种植修复后1 a,两组种植体边缘骨吸收量比较,差异无统计学意义(P>0.05)。结论上颌窦内提升术同期植骨与未植骨术后种植体松动度和边缘骨吸收量均在正常范围内,短期内临床效果未发现明显差异。

Objective To compare the effect of maxillary sinus elevation with simultaneous bone grafting and non-bone grafting.Methods A total of 48 patients(48 implants) who received maxillary sinus elevation combined with maxillary first molar implantation and regular review one year after repair in the First Affiliated Hospital of Zhengzhou University from August 2014 to August 2017 were selected as research object. The 24 implants that were filled with Bio-oss bone powder material in the maxillary sinus floor during the surgery were selected as bone grafting group, and the 24 implants that were not filled with bone grafting material in the maxillary sinus floor during the surgery were selected as non-bone grafting group. Three months after surgery, the implant stability quotient(ISQ) was measured by the implant mobility tester, and the influence of bone grafting or not on the implant looseness was analyzed. According to the actual height of bone resorption shown on the X-ray root tip film of the implants after one year of restoration, the marginal bone resorption of the implants was analyzed.Results Three months after surgery, there was no significant difference in ISQ between the two groups(P>0.05). One year of implant restoration, there was no significant difference in the amount of the marginal bone resorption of the implants between the two groups(P>0.05).Conclusion The implant mobility and marginal bone resorption were within the normal range after simultaneous bone grafting or non-bone grafting in maxillary sinus elevation, and there was no significant difference in clinical effect in the short term.
