实时荧光定量PCR技术检测妊娠晚期GBS感染的临床价值分析 被引量:9

Clinical Value of Real-time Fluorescence Quantitative Polymerase Chain Reaction in Detecting GBS Infection in the Third Trimester of Pregnancy

摘要 目的:探讨实时荧光定量PCR技术检测妊娠晚期B型链球菌(GBS)感染的临床价值。方法:选择2018年1月~12月在某院产检的200例单胎初产妇,取阴道拭子,分别使用普通细菌培养法与实时荧光定量PCR法检测是否合并GBS,记录入组孕妇GBS携带情况、分析细菌培养法与实时荧光定量PCR法检测GBS阳性率,并比较GBS阳性与阴性者母婴妊娠结局。结果:200例孕妇GBS携带率为10.50%(21/200);实时荧光定量PCR法检测GBS阳性率明显高于细菌培养法,差异有统计学意义(P<0.05);GBS阳性组孕妇不良妊娠结局总发生率、新生儿不良结局总发生率高于GBS阴性组,差异有统计学意义(P<0.05)。结论:GBS会增加孕妇与新生儿不良结局的发生,实时荧光定量PCR可快速准确筛查GBS,指导临床及时给予产妇干预措施,改善母婴结局。 Objective:To investigate the clinical value of real-time fluorescence quantitative PCR in the detection of Group B Streptococcus(GBS)infection in the third trimester of pregnancy.Methods:A total of200 cases of single primipara who underwent labor examination in a hospital from January to December 2018 were selected.Vaginal swabs were taken and GBS was detected by common bacterial culture method and realtime fluorescence quantitative PCR method respectively.The presence of GBS among the enrolled pregnant women was recorded,and the positive rate of GBS detected by bacterial culture method and real-time fluorescence quantitative PCR method were analyzed,and the maternal and infant pregnancy outcomes of positive and negative patients with GBS were compared.Results:The carrying rate of GBS in 200 pregnant women was 10.50%(21/200).The positive rate of GBS detected by real-time fluorescence quantitative PCR method was significantly higher than that by bacterial culture method,and the difference was statistically significant(P<0.05).The total incidence of adverse pregnancy outcomes and neonatal adverse outcomes in the GBS positive group were higher than those in the GBS negative group,with statistically significant differences(P<0.05).Conclusion:GBS can increase the occurrence of adverse outcomes in pregnant women and newborns.Real-time fluorescence quantitative PCR can rapidly and accurately screen GBS,guide clinical intervention measures for puerpera timely,and improve maternal and infant outcomes.
