以患者为中心的协同护理模式对老年糖尿病患者的影响

Effect of patient-centered collaborative nursing model on aged diabetic patients

目的 探讨以患者为中心的协同护理模式对老年糖尿病患者自我护理能力和血糖控制情况的影响。方法 2017年11月-2018年10月,根据患者入院编号,将138例老年糖尿病患者分为对照组和协同组两组。对照组患者采取常规护理方法,协同组患者采取以患者为中心的协同护理模式进行护理干预。从患者自我护理能力评分和血糖控制情况两方面,对护理效果进行评价。结果 协同组自我护理能力评分高于对照组,空腹血糖等血糖检测结果低于对照组,相比较,差异均有统计学意义(P<0.05)。结论 以患者为中心的协同护理模式有效提高了老年糖尿病患者的康复效果。

Objective To explore the effect of patient-centered collaborative nursing model on self-care ability and blood glucose controlling in aged diabetic patients. Methods From November 2017 to October 2018,138 elderly with diabetes were divided into control group and collaborative group according to their admission number.Patients in the control group were adopted routine nursing care,while patients in the collaborative group were given patient-centered collaborative nursing care.The nursing effect was evaluated from patients self-care ability score and blood glucose controlling. Results The score of self-care ability in the collaborative group was higher than those in the control group,while the fasting blood glucose level and other blood glucose tests were lower than those in the control group.The differences were statistically significant ( P <0.05). Conclusion Patient-centered collaborative nursing model can effectively improve the rehabilitation in elderly with diabetes.
