2015-2017年安溪县医院产妇HBV感染情况及新生儿首剂乙肝疫苗接种情况调查

Survey on HBV infection in maternal and the First dose of hepatitis B vaccination in Anxi county hospital 2015-2017

目的调查2015-2017年福建省安溪县医院HBs Ag阳性产妇和新生儿首诊接种的及时接种情况。方法选取2015-2017年在安溪县医院产科住院分娩的全部产妇14 760例和新生儿14 791例,对产妇年龄、常住地、有无乙肝接种史等信息分析乙肝感染情况,对所生新生儿的首诊接种率和及时率情况进行分析。结果①2015-2017年共收治住院分娩产妇14 760例,1221例产妇为HBs Ag阳性,感染率为8.27%;②HBs Ag阳性率无乙肝接种史产妇均明显高于有乙肝接种史产妇;③30岁以内,随着年龄增长,HBs Ag阳性率呈上升趋势。同时,在调查对象中,乙肝感染主要集中在25~35岁,感染率均>2.0%;④农村产妇HBs Ag阳性率均明显高于城镇产妇;⑤2015-2017年县医院首针接种率和及时率均高于全县水平,其中影响县医院首针接种及时率的原因主要是早产及体重过轻。结论安溪县医院2015-2017年产妇HBs Ag阳性率较高,应加强孕妇产前HBs Ag筛查和管理,完善新生儿首诊乙肝疫苗及时接种工作体系管理,切实提高母婴乙肝阻断效果。

Objective To investigate the first dose and timely vaccination of HBs Ag positive mothers and their newborns in anxi county hospital in 2015-2017.Methods All of the pregnant women who gave birth in the hospital and their newborns from 2015 to2017 were selected.The age,permanent residence,the history of hepatitis B vaccination and the status of hepatitis B infection of the mothers was analyzed.And the first dose vaccination rate and timely rate of the newborns were analyzed.Results①From 2015 to2017,a total of 14 760 pregnant women were in hospital for childbirth,and 1221 were positive of HBsag.Then infection rate was8.27%.②The HBs Ag positive rate of women without hepatitis B vaccination history was significantly higher than the HBs Ag positive rate of women with hepatitis B vaccination history.③Under the age of 30 years old,the positive rate of HBs Ag increased with the age.At the same time,hepatitis B infection was mainly concentrated between the ages of 25 and 35 years old,and the infection rate was greater than 2.0%.④The positive rate of HBs Ag among rural women was significantly higher than that of urban women.⑤From 2015 to 2017,the first dose vaccination rate and timely rate of county level hospitals were higher than that of the overall county.The reasons that affected the timely first injection rate of the county level hospitals were mainly premature delivery and underweight.Conclusion The maternal HBs Ag positive rate is high in 2015-2017.It is necessary to strengthen the prenatal screening and management of pregnant women,improve the management of hepatitis B vaccination and timely vaccination system for newborns,and effectively improve the blocking effect of hepatitis B in mothers and infants.
