乳腺钼钯、超声弹性成像与MRI临床用于诊断乳腺癌的对比分析 被引量:5

摘要 目的研究乳腺钼靶、超声弹性成像以及磁核共振(MRI)对乳腺癌的临床诊断价值。方法方便选取2016年9月—2018年5月期间该院收治65例疑似乳腺癌的患者作为研究对象。所有患者均进行乳腺钼靶、超声弹性成像以及MRI检查,分析3种检查方式对乳腺癌的临床诊断价值。结果65例患者中病理检查发现乳腺癌患者49例(75.38%),其中导管原位癌患者18例,黏液癌患者15例,腺样囊囊腺癌患者9例,髓样癌患者7例。其余16例均为良性肿瘤或肿块,占24.62%。乳腺钼靶以及超声弹性成像对乳腺癌诊断率分别为67.69%、69.23%,MRI诊断率为58.46%,乳腺钼靶及超声弹性成像对乳腺癌诊断率略高于MRI,差异有统计学意义(P<0.05)。乳腺钼靶和超声弹性成像诊断乳腺癌的诊断效能均差异无统计学意义(P>0.05);乳腺钼靶和超声弹性成像对乳腺癌特异度及阳性预测值均高于MRI,差异有统计学意义(P<0.05)。结论乳腺钼靶及超声弹性成像对乳腺癌诊断价值高于MRI,但单一乳腺钼靶或超声弹性成像对乳腺癌的诊断率仍然较低,两者联合检查可能会提高乳腺癌的诊断准确率。 Objective To compare the diagnosis value of mammary gland molybdenum target,ultrasonic elastography and MRI(magnetic resonance imaging)in breast cancer.Methods A total of 65 suspected breast cancer patients checked from September 2016 to May 2018 in our hospital were convenient selected.The mammary gland molybdenum target,ultrasonic elastography and MRI were taken to analyze its diagnosis value.Results The pathological examination indicated 49 breast cancer cases(75.38%)including 18 ductal carcinoma in situ cases,15 mucoid carcinoma cases,9 adenoid cystic carcinoma cases and 7 medullary carcinoma cases.The remaining 16 cases were proved as benign tumor or neoplasm(24.62%).The diagnosis rate of breast cancer based on the mammary gland molybdenum target,ultrasonic elastography and MRI was67.69%,69.23%and 58.46%,respectively.The diagnosis rate based on the mammary gland molybdenum target and ultrasonic elastography was higher than that based on MRI,the difference was statistically significant(P<0.05).The diagnosis efficiency based on the mammary gland molybdenum target and ultrasonic elastography was not significantly different(P>0.05).The diagnosis specificity and positive predictive value based on the mammary gland molybdenum target and ultrasonic elastography was higher than that based on MRI,the difference was statistically significant(P<0.05).Conclusion The mammary gland molybdenum target and ultrasonic elastography has a higher diagnosis value than MRI.However,the single mammary gland molybdenum target or ultrasonic elastography examination has an unfavorable diagnosis rate,which can be possibly improved by the combined examination mode.
