经尿道前列腺等离子剜除术和等离子切除术治疗前列腺增生的临床疗效对比

Comparison of Clinical Effects Between Transurethral Plasma Enucleation of the Prostate and Plasma Resection in the treatment of Benign Prostatic Hyperplasia

目的通过对前列腺增生患者分别采用经尿道前列腺等离子剜除术和等离子切除术,观察临床效果,探究两种手术方法的特点。方法从来该院诊治的2012年3月—2018年11月的前列腺增生患者中随机抽取78例,采用随机数字法随机分为剜除术组39例:实施经尿道前列腺等离子剜除术,电切术组39例:实施经尿道前列腺等离子电切术,比较两组手术情况及术后随访情况。结果剜除术组的切除组织量为(51.74±11.46)g,多于电切术组(t=11.684,P<0.05),手术出血量为(55.65±11.25)mL,少于电切术组(t=10.863,P<0.05),手术时间为(49.21±7.53)min,短于电切术组(t=13.861,P<0.05);剜除术组的术后并发症发生率为5.13%,低于电切术组(χ^2=12.031,P<0.05),术后残余尿量与电切术组差异无统计学意义(t=1.034,P>0.05),术后最大尿流速与电切术组差异无统计学意义(t=2.418,P>0.05),术后IPSS评分与电切术组差异无统计学意义(t=2.311,P>0.05)。结论经尿道前列腺等离子剜除术和等离子切除术相比切除组织更为彻底,可降低手术出血量、手术时间及术后并发症的发生,在治疗前列腺增生方面具有更多优势。

Objective To observe the clinical effect of transurethral plasma enucleation and plasma resection of prostate in patients with benign prostatic hyperplasia,and to explore the characteristics of the two surgical methods.Methods A total of 78 patients with benign prostatic hyperplasia treated in our hospital from January 2012 to March 2018 were randomly selected and divided into enucleation group(39 cases)with transurethral plasma enucleation of prostate and transurethral resection group(39 cases)with transurethral plasma resection of prostate.The operation and follow-up were compared between the two groups.Results The amount of resected tissue in the enucleation group was(51.74±11.46)g,which was higher than that in the electrotomy group(t=11.684,P<0.05).The amount of bleeding during the operation was(55.65±11.25)mL,which was less than that in the electrotomy group(t=10.863,P<0.05).And the operation time in the enucleation group was(49.21±7.53)min,which was shorter than that in the electrotomy group(t=13.861,P<0.05).The incidence of complications in the enucleation group was 5.13%,which was lower than that in the electrotomy group(χ^2=13.861,P<0.05).There was no significant difference in residual urine volume(t=1.034,P>0.05),maximum urinary flow rate(t=2.418,P>0.05)and IPSS score(t=2.311,P>0.05)between the electrosurgical group and the electrosurgical group.Conclusion Transurethral plasma enucleation of prostate is more thorough than plasma resection of prostate tissue,which can reduce the amount of bleeding,operation time and postoperative complications,and has more advantages in the treatment of benign prostatic hyperplasia.
