甘精胰岛素结合阿卡波糖治疗老年糖尿病的效果

Effect of Insulin Glargine Combined with Acarbose in the Treatment of Senile Diabetes Mellitus

目的分析老年糖尿病患者采用甘精胰岛素联合阿卡波糖的治疗效果。方法方便选择2016年1月—2018年10月期间该院收治的老年糖尿病患者62例作为该次研究对象,给与甘精胰岛素联合阿卡波糖药物治疗,对比患者治疗前后空腹血糖、空腹C肽、糖化血红蛋白、餐后2h血糖指标变化,比较治疗前后肝肾功能指标差异性,计算患者服药后不良反应发生率。结果患者治疗后空腹血糖(5.03±1.12)mmol/L、糖化血红蛋白(6.12±0.28)%、餐后2h血糖(7.59±1.43)mmol/L均有明显下降,空腹C肽指标(2.45±0.33)μg/L升高(t=15.6782、26.6025、19.7346、9.2322,P<0.05),且治疗后患者肌酐、尿素氮、总蛋白指标有所提高,尿酸、总胆固醇指标有所下降,但差异无统计学意义(t=1.0448、0.2475、0.5961、0.3512、0.5286,P>0.05)。患者服药后不良反应发生率为3.23%。结论老年糖尿病患者采用甘精胰岛素联合阿卡波糖药物治疗能够降低患者血糖指标,且对肝肾功能影响较小,不良反应少,值得临床应用。

Objective To analyze the therapeutic effect of insulin glargine combined with acarbose in elderly patients with diabetes mellitus.Methods A total of 62 cases of elderly diabetes patients admitted to our hospital from October 2016 to January 2016 were convenient selected as the research objects.Insulin glargine combined with acarbose was given to treat diabetes mellitus.The changes of fasting blood glucose,fasting C-peptide,glycosylated hemoglobin,2-hour postprandial blood glucose,and the difference of liver and kidney function indexes before and after treatment were compared.The incidence of adverse reactions was calculated.Results After treatment,fasting blood glucose(5.03±1.12)mmol/L,glycosylated hemoglobin(6.12±0.28)%,postprandial blood glucose(7.59±1.43)mmol/L decreased significantly.Fasting C-peptide index(2.45±0.33)μg/L increased(t=15.678 2,26.602 5,19.734 6,9.232 2,P<0.05).After treatment,and creatinine,urea nitrogen,total eggs,and white index of the patients were increased(t=15.678 2,26.602 5,19.734 6,9.232 2,P<0);uric acid and total cholesterol index were decreased,but the difference was not statistically significant(t=1.044 8,0.247 5,0.596 1,0.351 2,0.5286,P>0.05).The incidence of adverse reactions was 3.23%.Conclusion The treatment of insulin glargine combined with acarbose in elderly patients with diabetes mellitus can reduce the blood sugar index,and has little effect on liver and kidney function,and has less adverse reactions,which is worthy of clinical application.
