摘要
目的:分析超声和CT引导经皮穿刺活检在肺周围型新生物诊断中的应用。方法:回顾性总结因肺周围型新生物入院,行超声或CT引导经皮穿刺活检患者140例,依据穿刺方式分为超声引导组(67例)与CT引导组(73例),分析两组确诊率、并发症发生率、医疗费用和操作时间。结果:恶性肿瘤诊断率超声组(86.6%)高于CT组(76. 7%),并发症发生率超声组(9. 0%)小于CT组(15. 1%),但差异均无统计学意义。穿刺操作时间超声组[(22. 64±3. 18) min]小于CT组[(25. 45±3. 25) min],差异有统计学意义,超声组医疗费用小于CT组。结论:肺周围型新生物的超声与CT引导经皮穿刺活检均安全有效,超声引导更简单、快速和经济。贴近胸壁的肺脏周围型新生物,可首选超声引导;而非贴近胸壁的较深处病变,应首选CT引导。
Objective:To compare the clinical application of ultrasound or CT guided percutaneous lung biopsy in peripheral pulmonary neoplasm.Methods:The clinical characteristics and data were retrospectively analyzed in 140 patients with peripheral pulmonary neoplasm,who were performed with ultrasound guided or CT guided percutaneous pulmonary biopsy.These patients were divided into ultrasound guided group(67 cases) and CT guided group(73 cases).The accuracy of malignant tumor diagnosis,the incidence of complications,pulmonary biopsy time and hospitalization costs between the two groups were compared.Results:The accuracy of malignant tumor diagnosis in ultrasound guided group(86.6%) was higher than that of CT guided group(76.7%) and the incidence of complications in ultrasound guided group(9.0%) was less than that of CT guided group(15.1%),but there was no significant difference between two groups.Pulmonary biopsy time in ultrasound guided group[(22.64±3.18) min] was less than that of CT guided group[(25.45±3.25) min] and there was significant difference between two groups.The hospitalization costs of ultrasound guided group was less than that of CT guided group.Conclusion:Both ultrasound guided and CT guided percutaneous pulmonary biopsies are safe and effective in peripheral pulmonary neoplasm and ultrasound guidance is easier,faster and more ecomonical.Ultrasound guidance is preferred for peripheral pulmonary neoplasms which is close to the chest wall,but CT guidance is preferred for deep ones which is not close to the chest wall.
