振动反应成像系统(VRIxp)对肺切除手术后肺功能的预测作用

摘要 目的:研究振动反应成像系统(VRIxp)对肺叶切除患者术后肺功能的预测作用。方法:选取80例行肺叶切除术的患者,术前通过常规呼吸功能检查和振动反应成像系统(VRIxp)检查预测术后呼吸功能,术后1月复查常规呼吸功能。将术后实测呼吸功能值和预测呼吸功能值行统计学分析。比较两者间相关性和一致性。结果:VRIxp预测值和术后实测值结果配对t检验结果:第一秒用力呼气量占预计值百分比(FEV1%)、第一秒用力呼气量(FEV1L)、一氧化碳弥散量占预计值百分比(DLCO%)、一氧化碳肺弥散量(DLCO)的P值分别为0.79、0.27、0.59、0.68(P>0.05)。VRI预测值和术后实测值Pearson检验结果:FEV1%、FEV1L、DLCO%、DLCO ml/min/mmHg的相关系数r分别为:0.660、0.889、0.668、0.663,P值均小于0.000 1。VRI预测值和术后实测值的Bland-Altman一致性检验结果:FEV1%、FEV1L、DLCO%、DLCO差值的平均值和一致性界限分别为2.06%(0.37%~3.75%),-0.01 L(-0.03~0.04),-0.87%(-1.74%~0.01%)和-0.07 ml/min/mmHg(-0.43 ml/min/mmHg^0.28 ml/min/mmHg)。结论:VRIxp预测肺切除手术后呼吸功能结果与手术后实测呼吸功能之间有较强的相关性和一致性,可作为预测肺切除手术后呼吸功能的手段。 Objective:To investigate the predictive effect of vibration response imaging system(VRIxp) on respiratory function after lobectomy.Methods:Patients considered candidates for lung resection were studied.They underwent standard respiratory function and vibratory respond image(VRI) tests before surgery upon which projected postoperative respiratory function was calculated.Patients repeated respiratory function testing 1 month after surgery.The projected postoperative(PPO) respiratory function by VRI were compared to the actual postoperative respiratory function with a variety of statistic methodss.Results:Pared t test comparing PPO respiratory function by VRI and actual respiratory function(FEV1%,FEV1 L,DLCO% and DLCO) demonstrated P value were 0.79,0.27,0.59 and 0.68 respectively(P>0.05).Pearson test for PPO respiratory function by VRI and actual respiratory function(FEV1%,FEV1 L,DLCO% and DLCO) demonstrated r value of 0.660,0.889,0.668 and 0.663 respectively(P<0.000 1).Mean difference and limits of agreement using Bland-Altman test for PPO respiratory function by VRI and actual respiratory function FEV1%,FEV1 L,DLCO% and DLCO were 2.06%(0.37%~3.75%),-0.01 L(-0.03~0.04),-0.87%(-1.74%~0.01%) and-0.07 ml/min/mmHg(-0.43 ml/min/mmHg^0.28 ml/min/mmHg) respectively.Conclusion:VRI PPO respiratory function had good correlation and concordance with actual measured postoperative respiratory function.VRI might be an alternative test for lung function prediction.
