脾胃科中药临床药师开展药学服务的实践与探讨

Pharmaceutical Service of Traditional Chinese Medicine Clinical Pharmacists in Gastroen-terology Department

目的:基于中药临床药师日常药学服务工作实践,探讨脾胃科中药临床药师在中医院可实行的药学查房工作模式,为脾胃科中药临床药师工作提供借鉴。方法:介绍中药临床药师进入脾胃科开展的日常临床药学工作,通过典型案例的分析、总结,归纳临床药师参与脾胃科临床治疗的工作,重点分析中药师如何提高自身素质,协助中医师为患者制订个体化治疗方案,指导医师、护士和患者科学用药,减少药品不良反应,提高患者用药的依从性等。结果与结论:通过理论与实践相结合,中药临床药师应具备相关的中、西医药学知识,特别需要关注对症治疗、中西药间相互作用和药物之间的配伍禁忌等;具备良好的沟通能力,与医师协同发挥作用,共同参与临床工作,提供更优质的药学服务。中药临床药师在临床工作中担任着重要职责,开展药学服务不仅需借鉴临床药师模式,更需结合中医药特色,继承与创新相结合,将传统中医药思维与现代医药理念融会贯通,走具有中医药特色临床药学服务道路。

OBJECTIVE:Based on the practice of daily pharmaceutical service of traditional Chinese medicine (TCM) clinical pharmacists,to explore the pharmacy ward round mode that can be practiced by TCM pharmacists in gastroenterology department in TCM hospital,so as to provide reference for the work of TCM clinical pharmacists in gastroenterology department.METHODS:Introduction of daily clinical pharmacy work by TCM clinical pharmacists in gastroenterology department was conducted.Through the analysis of typical cases,the clinical pharmacists’ participation into to formulate the clinical treatment in gastroenterology department was summarized.The improvement of professional quality of TCM pharmacists,assistance for TCM clinicians to individualized treatment plans for patients,guidance of scientific medication for clinicians,nurses and patients,decrease of adverse drug reactions and improvement of patients’ medication compliance were analyzed.RESULTS & CONCLUSIONS:Through the combination of theory and practice,TCM clinical pharmacists should have relevant knowledge of Chinese and Western medicine,especially need to pay attention to the symptomatic treatment,interactions between Chinese and Western medicines,and incompatibility between drugs;and better communication skills should be attached more importance,TCM clinical pharmacists and physicians are suppose to work together to participate into clinical work and provide better pharmaceutical service.TCM clinical pharmacists play an important role in clinical work.The development of pharmaceutical service not only needs to learn from the clinical pharmacist model,but needs to combine the characteristics of TCM,inheritance and innovation,integrate TCM thinking and modern medical concepts,and the path of clinical pharmaceutical service with TCM characteristics should be taken.
