长期服用阿司匹林对脑出血患者凝血功能与生存状况的影响

Study on coagulation function and survival status of patients with long-term aspirin cerebral hemorrhage

目的探究长期服用阿司匹林对脑出血患者凝血功能以及其生存状况的影响。方法选取廊坊爱德堡医院神经外科自2010年9月至2017年9月收治的95例脑出血患者进行回顾性分析,根据研究期间是否长期服用阿司匹林分为试验组与对照组。试验组患者发病之前长期服用阿司匹林,对照组患者未服用阿司匹林。患者均于发病8 h内抽取静脉血进行血液检查,探究患者的凝血功能指标,采用美国国立卫生研究院卒中量表(NIHSS)对患者治疗前后神经功能进行评分,并对比2组患者术后持续出血情况、血肿体积增大以及死亡率。结果试验组患者血小板计数明显低于对照组,试验组的凝血酶时间、活化凝血酶时间明显高于对照组,差异均具有统计学意义(P<0.05)。试验组患者术后出血的发生率、血肿增大面积、死亡率以及NIHSS评分均明显高于对照组,差异有统计学意义(P<0.05)。结论长期服用阿司匹林可能会影响患者的凝血功能,严重的还会增加患者的死亡率。

Objective To investigate the effects of long-term use of aspirin on coagulation function and survival and prognosis in patients with cerebral hemorrhage.Methods A retrospective analysis of 95 patients with cerebral hemorrhage who were treated in our hospital from September 2010 to September 2017 was performed.The long-term use of aspirin during the study period was divided into experimental group and control group.The experimental group took long-term use of aspirin before onset,the control group did not take aspirin.The patient’s venous blood was taken for blood test within 8 h after the onset of the disease,and the patient’s blood coagulation function index was explored.The United States National Institute of Health stroke scale was used to evaluate the neurological function of patients before and after treatment,and the continuous bleeding,enlarged hematoma volume and mortality were compared between the two groups.Results The platelet count in the experimental group was significantly lower than that in the control group,and the thrombin time and activated thrombin time in the experimental group were significantly higher than those in the control group(P<0.05).The incidence of hemorrhage,enlarged hematoma area,mortality and NIHSS score in the experimental group were significantly higher than those in the control group(P<0.05).Conclusion Long-term use of aspirin may affect the patient’s coagulation function,which severely increase the mortality of patients.
