住院医师培训质量的影响因素及对策研究

Influencing Factors and Countermeasures of Training Quality of Resident Doctors

目的调查住院医师培训质量影响因素并进行相关提高质量对策分析。方法选择2015年5月-2018年5月莆田学院附属医院与莆田市第一医院住院医师规范化培训学员184人以及带教老师112人为对象,所有学员与带教老师均接受特定问卷调查。结果学员基本可以保证每周接受1~4次技能操作和查房带教,而病例讨论、学术活动、督查考试则保证在每周1~2次频率之间;学员对培训内容、培训效果与总体培训满意度基本超过62.0%,分别为66.3%,62.5%与67.4%,对于培训设施满意度情况欠佳,为33.2%。使用Logistic回归分析培训满意度影响因素结果显示学员态度、带教老师师德、带教水平、培训内容、培训效果与培训满意度正相关,培训方式和培训设施与培训满意度负相关。结论住院医师培训应该加强培训相关设施建设,增加病例讨论以及学术活动等相关带教内容以进一步提高培训质量。

Objective To investigate the influencing factors of training quality of resident doctors, and to analyze the related countermeasures that improve quality. Methods A total of 184 students who were given standardized training by resident doctors and their 112 teachers who were from the affiliated hospitals of Putian University and Putian First Hospital during the period from May 2015 to May 2018 were enrolled. All students and teachers was given specific questionnaires. Results Skill operations,ward round and teaching for 1 to 4 times a week could be basically guaranteed for the students. The frequencies of case discussions, academic activities and supervision exams were guaranteed between within 1 to2 times a week. The satisfaction rates of students with training content,training effect and overall training were basically more than 62.0%(66.3%,62.5% and 67.4%). The satisfaction with training facilities was not good(33.2%). Logistic regression analysis showed that in the influencing factors of training satisfaction, students attitude, teacher’s ethics, teaching level,training content and training effect were positively correlated with training satisfaction. The training methods and training facilities were negatively correlated with the training satisfaction. Conclusion In the training for resident doctors, construction of training-related facilities should be strengthened. The related teaching contents such as case discussions and academic activities should be increased to further improve training quality.
