颅脑损伤患者家属疾病不确定感现状及其影响因素分析

Status and influencing factors of illness uncertainty among family members of brain injury patients

目的了解颅脑损伤患者家属疾病不确定感现状和应对方式现状,探讨家属疾病不确定感的影响因素。方法采用便利抽样法,选择2017年5月—2018年8月入住河南省某医院神经外科颅脑损伤患者家属109名为研究对象。采用一般资料调查表、疾病不确定感家属量表、简易应对方式量表进行问卷调查。颅脑损伤患者家属的疾病不确定感与应对方式的相关性采用Pearson相关分析,疾病不确定感影响因素采用多元线性回归分析。结果颅脑损伤患者家属疾病不确定感评分为(90.79±27.14)分;积极应对方式评分为(19.56±10.01)分;消极应对方式评分为(10.16±5.94)分。多元线性回归分析显示,患者的格拉斯哥昏迷(GCS)评分、住院天数、家属的学历及慢性疾病情况是家属疾病不确定感得分的主要影响因素(P<0.05)。颅脑损伤患者家属疾病不确定感评分与积极应对方式评分呈负相关(r=-0.309,P<0.01)。结论颅脑损伤患者家属疾病不确定感处于中等水平,并受到患者GCS评分、住院天数、家属的学历及慢性疾病情况影响,而积极的应对方式具有降低家属疾病不确定感的作用。

Objective To understand the current status of illness uncertainty among family members of brain injury patients and its coping style and to explore the influencing factors of illness uncertainty of family members.Methods From May 2017 to August 2018,we selected 109 family members of brain injury patients as subjects in Neurosurgery Department at a hospital in He'nan by convenient sampling method.Questionnaire survey was carried out with the general information questionnaire,Mishel Uncertainly in Illness Scale for Family Member(MUIS-FM)and Simplified Coping Style Questionnaire(SCSQ).Pearson correlation was used to analyze the correlation between illness uncertainty and the coping style among family members of brain injury patients.Multiple linear regression was used to analyze the influencing factors of illness uncertainty.Results Among family members of brain injury patients,the scores of illness uncertainty,positive coping style and negative coping style were(90.79±27.14),(19.56±10.01)and(10.16±5.94)respectively.Multiple linear regression analysis showed that the main influencing factors of illness uncertainty among family members of brain injury patients included the score Glasgow Coma Scale(GCS)of patients,hospital stay,education level of family members and chronic disease(P<0.05).The score of illness uncertainty had a negative correlation with the score of positive coping style among family members of brain injury patients(r=-0.309,P<0.01).Conclusions Family members of brain injury patients have a medium level of illness uncertainty which influenced by the score GCS,hospital stay,education level of family members and chronic disease.Positive coping style has a role in decreasing the illness uncertainty of family members.
