下肢康复机器人训练对脑卒中患者步行能力的影响 被引量:2

Effect of robot-assisted gait training on the gait ability of stroke patients

摘要 目的观察下肢康复机器人辅助步行训练对脑卒中偏瘫患者步行能力的影响。方法2015年1月至2016年10月,60例脑卒中患者随机分为机器人组(n=30)和对照组n=30)。两组均在接受常规训练基础上,机器人组予下肢机器人辅助步行训练,对照组予人工辅助步行训练。训练时间均为每次30min,每周5次,连续训练8周。训练前后分别采用机器人评估系统评定下肢屈髋肌力(FHF)、6min步行距离(6MWD)、10m步行时间(MWS)与FUgl-Meyer下肢运动功能评分(FMA-LE)评定步行能力。结果训练前,两组间一般资料及各项指标差异无统计学意义(P>0.05)。训练后,两组步行能力均明显提高(P<0.05)。其中,机器人组FHF、6MWD、MWS、FMA-LE分别为(51.53±22.24)Nm、(80.50±15.62)m、(45.54±29.49)s和(21.81±7.77)分,与对照组的(43.65±24.94)Nm、(66.44±19.81)m、(58.89±21.76)s和(15.13±8.67)分比较,差异均有统计学意义(P<0.05)。结论下肢康复机器人辅助步行训练能够有效提高脑卒中偏瘫患者的步行能力。 Objectives To explore the effect of robot-assisted gait training (RAGT) on the gait ability in patients with hemiplegia after stroke. Methods From January 2015 to October 2016, a total of 60 patients with hemiplegia were randomly divided into the RAGT group (n=30) and control group (n=30). Both groups received conventional rehabilitation, while the RAGT group was given robot-assisted gait training and control group was given manually assisted gait training 30 minutes a day, 5 days a week for 8 weeks. L-FORCE/HIP/ Flex (FHF), 6-minutes walking distance (6MWD), maximum walking speed (MWS) and Fugl-Meyer Assessment of legs (FMA-LE) were evaluated before and after training. Results There was no significant difference in general information and indexes between the two groups before training (P > 0.05). The scores improved in both groups after training(P< 0.05). The FHF, 6MWD, MWS, and FMA-LE of RAGT group was (51.53 ± 22.24)Nm,(80.50 ± 15.62)m,(45.54 土 29.49)s and (21.81 ± 7.77 ),respectively, which were significantly better than the control group [(43.65 ±24.94)Nm,(66.44 ± 19.8l)m ,(58.89 ± 21.76)s and (15.13 ± 8.67), P < 0.05]. Conclusions Robot-assisted gait training can effectively improve the gait ability of stroke patients with hemiplegia.
