AIDET沟通模式联合强化疼痛管理在产科分娩镇痛护理中的应用

Application of AIDET Communication Mode Combined with Enhanced Pain Management in Obstetric Labor Analgesia Nursing

目的:探讨AIDET沟通模式联合强化疼痛管理在产科分娩镇痛护理中的应用效果。方法:将106例产妇随机分为研究组和对照组各53例,研究组接受AIDET沟通模式联合强化疼痛管理干预,对照组在常规围生期护理基础上实施强化疼痛管理。比较两组焦虑自评量表(SAS)评分、疼痛视觉模拟评分法(VAS)评分、中文简化版分娩自我效能量表(CB-SEI-C32)评分和两组孕妇分娩结局。结果:产程开始时至产后2 h,两组孕妇SAS、VAS评分均呈先升高后降低趋势,研究组孕妇产程起始时、宫口扩张3 cm及产后2 h SAS评分与VAS评分均低于对照组(P <0. 05);产前1 d,两组CB-SEI-C32分量表及总分均较入院后第1天升高(P <0. 05),且研究组产前1 d的CB-SEI-C32总分及其分量表得分均高于对照组(P <0. 05);研究组自然分娩率高于对照组(P <0. 05);研究组总产程、产后24 h泌乳量评分、新生儿评分均高于对照组(P <0. 05)。结论:AIDET沟通模式联合强化疼痛管理可提高孕妇自我效能,有效降低产妇焦虑水平和分娩疼痛程度,提高分娩质量。

Objective: To explore the application effect of AIDET communication mode combined with enhanced pain management in obstetric labor analgesic nursing. Methods: A total of 106 cases were randomly divided into study group and control group,53 cases each. The study group received AIDET communication mode combined with enhanced pain management intervention. The control group underwent intensive pain management on the basis of routine perinatal nursing. The scores of SAS,VAS,CB-SEI-C32 and birth outcomes were compared between the two groups. Results: From the beginning of labor to 2 hours postpartum,the SAS and VAS scores of the two groups of pregnant women showed an increasing trend and then a decreasing trend. At the beginning of labor,the uterine dilatation was 3 cm,and the SAS and VAS scores at 2 hours postpartum in the study group were lower than those in the control group( P < 0. 05). The total subscale score of CBSEI-C32 and the total score of CB-SEI-C32 on the first day of antenatal delivery were higher in the two groups than on the first day after admission( P < 0. 05),and the total subscale score of CB-SEI-C32 and the total score of CB-SEI-C32 on the first day of antenatal delivery in the study group were higher than those in the control group( P < 0. 05). The natural delivery rate in the study group was higher than that in the control group( P < 0. 05). The total length of labor,24 hours postpartum milk production score and neonatal score in the study group were all higher than those in the control group( P < 0. 05). Conclusion: AIDET communication mode combined with enhanced pain management can improve maternal self-efficacy,effectively reduce maternal anxiety level and pain degree,and improve the quality of delivery.
