摘要
目的探索三级专科医院应用DRGs对医疗质量评价的实践及效果,为三级肿瘤专科医院质量管理提供新思路,为客观的用DRGs评价医院各科室医疗质量提供良好的理论依据。方法本研究依托上海联众DRGs数据分析平台结合医院病案首页,应用综合指数法,从业务能力、学科水平、疑难诊治程度、医疗服务效率及医疗安全五个方面对医院各科室医疗质量进行分析评价。结果DRGs按综合指数法评价医疗质量排名,E、A、B、T、Q、U、Y科室在各诊疗系统内医疗质量好于其他科室;D、I、J、S、W科室在各诊疗系统排名靠后,说明科室医疗质量各维度出现不同程度的问题。结论基于DRGs评价指标应用综合指数法能够客观评价各科室医疗质量,准确地反映各科室医疗质量的优势和不足,也可依据评价结果对各科室存在的主要问题制定针对性医疗质量改进措施,提高医、护、患三方满意度。
Objective Exploring the practice and effect of DRGs in the evaluation of medical quality in tertiary tumor specialized hospitals, providing new ideas for the quality management of tertiary specialized hospitals, and providing a good theoretical basis for the objective evaluation of hospitals and departments with DRGs. Methods Based on the Shanghai Lianzhong DRGs data analysis platform and the hospital medical record homepage application comprehensive index method, this study analyzes and evaluates the medical quali. ty of hospital departments from five aspects: business ability, subject level, difficulty diagnosis and treatment, medical service efficiency and medical safety. Results DRGs ranked E, A, B, T, Q, U, Y in medical quality according to the comprehensive index method. The medical quality of D, I, J, S and W departments ranked lower than that of other departments in the medical system, which indicated that there were problems in different dimensions of medical quality of departments. Conclusion Based on the DRGs evaluation index system, the comprehensive index method can objectively evaluate the medical quality of each department, accurately reflect the advantages and disadvantages of the medical quality of each department, also formulate targeted medical quality improvement measures based on the evaluation results for the main problems existing in each department. It improves the satisfaction of doctors, nurses and patients.
