高频彩超引导下改良法细针负压抽吸活检与无负压活检用于甲状腺结节的诊断价值比较

Comparison of diagnostic value of modified fine needle aspiration biopsy and non-negative pressure biopsy under the guidance of high frequency color Doppler ultrasound in thyroid nodules

目的比较高频彩超引导下改良法细针负压抽吸活检与无负压活检对甲状腺结节的诊断效果。方法选取2015年11月—2018年11月阜宁县人民医院收治的甲状腺结节患者325例,其中接受彩超引导下无负压细针穿刺活检的甲状腺结节患者160例(对照组),同期接受彩超引导下改良法细针负压抽吸活检的甲状腺结节患者165例(观察组)。对比2组标本获得量、标本满意度、诊断成功率、术中及术后出现的并发症。结果观察组标本满意度高于对照组,有确定意义细胞学诊断结果的比例高于对照组,差异有统计学意义(P < 0. 05 )。2组灵敏度、特异度、阳性预测值、阴性预测值及诊断准确性之间的差异无统计学意义(P >0.05),并发症发生率的差异无统计学意义(P>0.05)。结论改良法细针负压抽吸活检操作简单,具有更高标本满意度及更高有确定意义细胞学诊断结果比例,且安全性较高,但其在甲状腺结节良恶性鉴别诊断方面与无负压细针穿刺活检无显著差异。

Objective To compare the diagnostic effect of modified negative pressure fine needle aspiration biopsy and non - negative pressure fine needle aspiration biopsy guided by high frequency color Doppler ultrasound in thyroid nodules. Methods A total of 325 patients with thyroid nodules admitted to Funing County Peopled Hospital from No vember 2015 to November 2018 were selected. Among them, 160 patients ( control group) received ultrasound - guided fine needle aspiration biopsy without negative pressure, and 165 patients ( observation group) received modified negative pressure fine needle aspiration biopsy under the guidance of color Doppler ultrasound. The acquisition of two groups of specimens, specimen satisfaction, diagnostic success rate, intraoperative and postoperative complications were compared. Results The satisfaction of the observation group was higher than that of the control group, and the proportion of definite cytological diagnosis results was higher than that of the control group, with statistical significance (P <0. 05 ). There was no significant difference in sensitivity,specificity, positive predictive value, negative predictive value and diagnostic ac curacy between the two groups ( P > 0.05 ). There was no significant difference in the incidence of complications between the two groups ( P > 0.05 ). Conclusions The modified negative pressure fine needle aspiration biopsy is simple in op eration ,has higher sample satisfaction, higher definite cytological diagnosis rate and higher safety, but in the differential diagnosis of benign and malignant thyroid nodules, there is no significant difference between the modified negative pres sure fine needle aspiration biopsy and non - negative pressure fine needle aspiration biopsy.
