摘要
目的比较X线、CT三维重建及MRI 3种影像学检查方法对复杂性胫骨平台骨折的临床诊断价值。方法选择2015年1月-2017月6月在江苏省第二中医院影像科接受治疗的55例复杂性胫骨平台骨折患者,所有患者均在术前接受X线、CT三维重建与MRI检查,并经内固定术证实,比较X线、CT三维重建与MRI的检查结果。结果 55例复杂性胫骨平台骨折患者经X线、三维CT重建及MRI检查后术前诊断分型与手术结果分型的相符率分别为8.18%(43/55)、90.91%(50/55)、85.45%(47/55);不同检查方式间诊断与手术结果的分型相符率差异不大,均无统计学意义(P>0.05)。结论 X线、三维CT及MRI均能够为复杂性胫骨平台骨折的诊断提供有效资料,多种影像学检查方法互相结合、互相补充能够更加有效地提高复杂性胫骨平台骨折诊断的准确率。
Objective To compare the clinical diagnostic value of X - ray, multi - slice spiral three - dimensional (3D) CT reconstruction and MRI in complex tibial plateau fractures. Methods A total of 55 patients with complex tibial plateau fractures in our hospital for treatment from January 2015 to June 2017 were selected for this study. All patients underwent X - ray, 3D CT reconstruction and MRI examination before operation, and were confirmed by internal fixation. The findings of X - ray, 3D CT reconstruction and MRI were compared. Results The coincidence rates of preoperative diagnostic classification and surgical results were 8. 18%(43/55 ), 90. 91%(50/55) and 85.45%(47/55 ), respec tively in 55 patients with complex tibial plateau fractures after X - ray, 3D CT reconstruction and MRI examination. There was no significant difference in the diagnosis rates among different examinations and the results of surgery (P >0.05). Conclusions X - ray, 3D CT reconstruction and MRI can all provide effective diagnosis and classification basis for complex tibial plateau fractures, but each examination has its own merits and demerits. A variety of imaging examina tion combined with each other and supplemented each other can more effectively improve the accuracy of diagnosis for complex tibial plateau fracture, and provide higher diagnostic value for clinical practice.
