甲状腺功能减退孕妇TSH浓度与分娩结局的相关性分析

Correlation Analysis of TSH Concentration and Delivery Outcome in Hypothyroid Pregnant Women

目的研究分析甲状腺功能减退孕妇血清促甲状腺素(TSH)浓度与分娩结局的相关性。方法研究对象选取为2016年4月~2018年8月本院收治的79例甲状腺功能减退孕妇,设为观察组,选取同期的100例正常孕妇作为对照组,对比两组孕妇分娩前、产后1周的TSH水平,对比两组孕妇的产科并发症情况,并统计对比两组分娩结局及新生儿状况。结果观察组分娩前、产后1周的TSH水平均显著高于对照组,差异显著(P <0.05);观察组的总体产科并发症发生率均显著高于对照组(P <0.05);观察组的低体重儿、胎儿窘迫发生率均显著高于对照组,新生儿体重及Apgar评分均显著低于对照组(P <0.05)。结论孕妇甲状腺功能减退可能是导致产科并发症和不良妊娠结局的重要原因,应当加强血清促甲状腺素水平的监测,尽早发现尽早治疗。

Objective To study and analyze the correlation between serum thyrotropin (TSH) concentration and delivery outcome in hypothyroid pregnant women. Methods 79 cases of hypothyroid pregnant women admitted to our hospital from April 2016 to August 2018 were selected as the observation group, and 100 normal pregnant women in the same period were selected as the control group. The TSH levels before delivery and one week after delivery were compared between the two groups. The obstetric complications of the two groups were compared, and the delivery outcomes and neonatal status of the two groups were statistically compared. Results TSH levels in the observation group were significantly higher than those in the control group before delivery and one week after delivery (P < 0.05). The overall incidence of obstetric complications in the observation group was significantly higher than that in the control group (P < 0.05). The incidence of low birth weight infants and fetal distress in the observation group were significantly higher than those in the control group, and the neonatal weight and Apgar score were significantly lower than those in the control group (P < 0.05). Conclusion Hypothyroidism in pregnant women may be an important cause of obstetric complications and adverse pregnancy outcomes. Monitoring of serum thyrotropin level should be strengthened so as to detect and treat as soon as possible.
