照顾者反应对接受抗反转录病毒治疗艾滋病患儿照顾者负担的影响

Effect of caregivers reaction on the burden of caregiver of AIDS children receiving antiretroviral therapy

目的分析照顾者反应对接受抗反转录病毒治疗(ART)艾滋病(AIDS)患儿的照顾者负担的影响。方法便利抽取在门诊接受国家免费ART AIDS患儿的主要照顾者,采用照顾者反应评估量表(CRA)、照顾者负担量表(ZBI)调查其在照顾上的反应及负担,使用Logistic回归分析照顾者反应对照顾者负担的影响。结果共入组112例接受ART AIDS患儿的照顾者为研究对象。(1)照顾者负担总分为(31.05±17.89)分;其中轻度负担占25.9%,中等及以上占57.2%,不同特征的AIDS患儿及照顾者,其照顾者负担差异无统计学意义(P> 0.05);(2)照顾者反应自尊得分最高(3.86±0.52),其余消极结果得分从高到低依次为经济问题维度、时间安排受打扰、健康问题、家庭支持维度;(3)照顾者反应中的健康问题、经济问题、家庭支持缺乏是影响AIDS患儿照顾者负担的主要因素。结论接受ART AIDS患儿的照顾者负担较重,可能与照顾者反应包括健康问题、经济问题及缺乏家庭支持相关,因此在提高患儿服药依从性过程中应重视上述因素。

Objective To investigate the effect of the caregivers response on the burden on caregiver of AIDS children receiving antiretroviral therapy(ART). Methods Eligible caregivers for AIDS children receiving ART were enrolled in the study. The Caregiver Reaction Assessment(CRA)and Zarit Caregiver Burden Interview(ZBI)were applied to investigate caregivers′ reaction and burden respectively. Results 112 caregivers were involved in this study. The total burden level score of all caregivers is 31.05 ± 17.89;with 25.9% of them having light burden and 57.2% medium or high burden. The burden level scores of caregivers were not significantly different according to the features of AIDS children or their caregivers(P > 0.05). In regard of caregivers score of self-esteem was the highest(3.86 ± 0.52),sequentially followed by economic problem,disturbed time reactions,the main factors affecting caregivers burden included health problem,economic problem and lack of family support. Conclusions Heavy burden of caregiver of AIDS children receiving ART is possibly associated caregivers problems,economic problems,and lack of family support. Therefore,these factors should be carefully dealt with when we aim to enhance the coherence of ART treatment for AIDS children.
