双氯芬酸钠栓对结肠癌患者术后疼痛及炎症反应的影响

Effect of diclofenac sodium suppositories on postoperative pain and inflammatory response in patients with colon cancer

目的观察双氯芬酸钠栓对腹腔镜结肠癌根治术后患者疼痛、炎症反应及术后恢复情况的影响。方法选择2017年1月至2018年6月阜外华中心血管病医院收治的结肠癌患者62例,随机分为两组。观察组应用围术期双氯芬酸钠栓纳肛及术后静脉镇痛泵自控镇痛(PCIA),对照组仅应用PCIA。在术前(T0)、术后2h(T1)、术后6h(T2)、术后12h(T3)、术后24h(T4)、术后48h(T5)等时间点分别观察两组的镇痛效果,炎症因子水平以及术后肠道功能恢复情况。结果术后T1、T2、T3、T4时观察组镇痛效果和炎症因子水平均优于对照组(P<0.05);观察组肠道功能恢复和术后康复情况也优于对照组(P<0.05)。结论双氯芬酸钠栓超前镇痛联合PCIA能减轻结肠癌根治术患者的术后疼痛和炎症反应,促进术后恢复,值得临床推广应用。

Objective To observe the effect of preemptive analgesia with diclofenac sodium suppositories on pain,inflammation and postoperative recovery in patients with colon cancer after radical operation.Methods Sixty-two patients with sigmoid colon cancer in Fuwai Central China Cardiovascular Hospital from January 2017 to June 2018 were randomly divided into two groups.The patients in the observation group were treated with diclofenac sodium suppositories and postoperative analgesia by patient controlled intravenous analgesia(PCIA),and the control group only used PCIA.The analgesia effect,inflammatory factors and postoperative recovery of the two groups were observed before operation(T0),2 hours after operation(T1),6 hours after operation(T2),12 hours after operation(T3),24 hours after operation(T4)and 48 hours after operation(T5).Results The analgesia effect and the levels of inflammatory factors of the observation group was significantly better than those of the control group at T1,T2,T3 and T4 time point(P<0.05);the intestinal function recovery and postoperative rehabilitation of the observation group were also better than those of the control group(P<0.05).Conclusions Preemptive analgesia with diclofenac sodium suppositories can alleviate the postoperative pain and the inflammatory reaction,promote the postoperative rehabilitation in patients with colon cancer undergoing radical operation,which is worthy of clinical application.
