16排螺旋CT对结缔组织相关性肺间质病变的诊断价值 被引量:2

Application value of 16 slice spiral CT in the diagnosis of connective tissue associated interstitial lung disease

摘要 目的探讨16排螺旋CT用于结缔组织相关性肺间质病变诊断的价值。方法选取临汾市第二人民医院2016年3月至2018年4月确诊的108例结缔组织病患者作为结缔组织病组,并选取同期接受常规体检的50例健康人群为对照组,两组均接受16排螺旋CT高分辨扫描与肺密度测定,对比不同CT检查方式检出肺间质病变的情况,同时对比两组经螺旋CT肺密度的测定结果。结果108例结缔组织病患者经肺高分辨扫描分期发现,1期检出率最高,为47.22%,其次为0期(28.70%)、2期(15.74%)、3期(8.33%);108例结缔组织病患者经CT检查其肺密度发现,B区检出率最高,为67.59%,其次为C区(21.30%)及A区(11.11%);多层螺旋CT肺密度测定对结缔组织相关性肺间质病变总检出率高于肺高分辨扫描(P<0.05),且结缔组织病组多层螺旋CT肺密度检测的阳性检出率高于对照组(P<0.05)。结论16排螺旋CT诊断结缔组织相关性肺间质病有理想的应用价值,且多层螺旋CT肺密度检测诊断的敏感性优于肺部高分辨扫描,因其具有的可重复性、客观性、无创性,可作为结缔组织相关性肺间质病的首选诊断方法。 Objective To evaluate the value of 16 slice spiral computed tomography(CT)in the diagnosis of connective tissue associated interstitial lung disease.Methods One hundred and eight patients with connective tissue disease diagnosed in the Second People’s Hospital of Linfen from March 2016 to April 2018 were selected as connective tissue disease group,and 50 healthy people who received regular physical examination during the same period were selected as control group.Both groups received 16 slice spiral CT high resolution scan and lung density determination.The condition of pulmonary interstitial lesions checked by different CT examinations were compared.And the lung densities measured by spiral CT were compared between the two groups.Results Among the 108 patients with connective tissue disease,the highest detection rate was in the 1 stage(47.22%),followed by 0 stage(28.70%),2 stage(15.74%)and 3 stage(8.33%)according to high-resolution lung scan.And the detection rate of B area was the highest(67.59%),followed by C area(21.30%)and A area(11.11%)according to lung density determined by CT.The detection rate of connective tissue associated interstitial lung disease by multi-slice spiral CT pulmonary density determination was higher than that by high-resolution lung scan(P<0.05);and the positive rate of multislice CT lung density detection in connective tissue disease group was higher than that in control group(P<0.05).Conclusions 16 slice spiral CT has an ideal application value in the diagnosis of connective tissue associated interstitial lung disease.The sensitivity of multislice CT lung density detection is more ideal than that of high-resolution lung scan.Because of its repeatability,objectivity and noninvasive features,16 slice spiral CT can be used as the first choice for diagnosis of connective tissue associated interstitial lung disease.
