外剥内扎术和吻合器痔上黏膜环切术治疗混合痔的效果及近远期并发症比较

Comparison of efficacy and short-term and long-term complications between Milligan-Morgan and procedure of prolapse and hemorrhoids for mixed hemorrhoids

目的对比分析外剥内扎术(M-M)、吻合器痔上黏膜环切术(PPH)治疗混合痔的效果及近远期并发症发生情况。方法选择大同市中医医院2015年3月至2017年2月收治的混合痔患者102例,经随机数字表法分为研究组和对照组,每组51例。对照组予以M-M手术,研究组予以PPH手术,记录两组手术指标及近远期并发症发生情况。结果研究组手术时间、术中出血量、术后疼痛持续时间、术后住院时间均少于对照组,差异有统计学意义(P<0.05);研究组术后近期并发症发生率(3.92%)低于对照组(21.57%),差异有统计学意义(P<0.05);两组术后均随访12个月,随访成功率为100.00%,研究组远期并发症发生率(7.84%)低于对照组(27.45%),差异有统计学意义(P<0.05)。结论应用PPH手术治疗混合痔可显著降低患者的近远期并发症发生率,有利于保障患者的预后及生活质量。

Objective To compare the effect and the short-term and long-term complications of patients with mixed hemorrhoids after Milligan-Morgan(M-M)and procedure for prolapse and hemorrhoids(PPH).Methods A total of 102 patients with mixed hemorrhoids admitted to Datong Hospital of Traditional Chinese Medicine from March 2015 to February 2017 were selected,and they were divided into study group and control group according to random number table method,with 51 cases in each group.Patients in control group were treated by M-M,and patients in study group were treated by PPH.The surgical indexes and short-term and long-term complications of the two groups were recorded.Results The operation time,intraoperative bleeding volume,postoperative pain duration and hospital stay in study group were shorter and less than those in control group,and the difference was statistically significant(P<0.05).The incidence of short-term complications in study group(3.92%)was lower than that in control group(21.57%),and the difference was statistically significant(P<0.05).Both groups were followed up for 12 months,and the success rate of follow-up was 100.00%.The incidence of long-term complications in study group(7.84%)was lower than that in control group(27.45%),and the difference was statistically significant(P<0.05).Conclusions The use of PPH in the treatment of mixed hemorrhoids can significantly reduce the incidence of short-term and long-term complications,which is conducive to the prognosis and quality of life.
