经内镜逆行性胰胆管造影术治疗胆总管结石的临床效果

Clinical effect of endoscopic retrograde cholangiopancreatography in the treatment of common bile duct stones

目的研究经内镜逆行性胰胆管造影术治疗胆总管结石的临床效果。方法选取2015年2月至2018年9月我院收治的74例胆总管结石患者,根据手术方案不同将其分为对照组与试验组,各37例。对照组给予开腹胆总管探查术,试验组给予内镜逆行性胰胆管造影术。比较两组的治疗效果。结果试验组的结石清除率高于对照组,住院时间及手术时间短于对照组,住院费用、并发症总发生率低于对照组(P<0.05)。术后7、14d,两组VAS评分均降低,且试验组低于对照组(P<0.05)。两组复发率无显著差异(P>0.05)。术后3个月,两组躯体疼痛、生理功能、情感职能、精神健康、社会功能、总体健康、生理职能、活力评分均升高,且试验组高于对照组(P<0.05)。结论经内镜逆行性胰胆管造影术治疗胆总管结石效果显著,可缓解患者疼痛,改善其生活质量并加快术后康复进程。

Objective To study the clinical effect of endoscopic retrograde cholangiopancreatography in the treatment of common bile duct stones. Methods A total of 74 patients with common bile duct stones in our hospital from February 2015 to September 2018 were selected and divided into control group and experimental group according to the different operation plan, with 37 cases in each group. The control group received open common bile duct exploration and the experimental group received endoscopic retrograde cholangiopancreatography. The therapeutic effects of the two groups were compared. Results The stone clearance rate of the experimental group was higher than that of the control group, the hospital stay and operation time were shorter than those of the control group, and the hospital expenses and total incidence of complications were lower than those of the control group (P<0.05). On the 7th and 14th day after operation, the VAS scores of both groups decreased, and those of the experimental group were lower than the control group (P<0.05). There was no significant difference in the recurrence rate between the two groups (P>0.05). Three months after operation, the scores of physical pain, physiological function, role emotional, mental health, social function, general health, role physical and vitality of the two groups increased, and those of the experimental group were higher than the control group (P<0.05). Conclusion Endoscopic retrograde cholangiopancreatography is effective in the treatment of common bile duct stones. It can relieve pain, improve the quality of life and accelerate the recovery process of patients.
