腹腔镜胆囊切除术治疗急性发作期胆囊炎的临床效果

Clinical effect of laparoscopic cholecystectomy on acute episodecholecystitis

目的探究腹腔镜胆囊切除术治疗急性发作期胆囊炎的临床效果。方法选取2014年1月至2018年12月我院收治的80例急性发作性胆囊炎患者,将其中39例行开腹胆囊切除术的患者设为开腹组,41例行腹腔镜胆囊切除术的患者设为腔镜组。比较两组的治疗效果。结果腔镜组手术时间、术后肛门排气时间、下床时间和住院时间均明显短于开腹组,术中失血量明显少于开腹组(P<0.05)。腔镜组并发症总发生率明显低于开腹组(P<0.05)。手术后1d,两组患者PRI感觉评分、PRI情感评分、VAS评分、PPI评分和总分均降低,且腔镜组低于开腹组(P<0.05)。手术后1周,两组患者主观症状、心理情绪状态、生理功能状态、社会活动状态、消化系统专科情况评分及总分均升高,且腔镜组高于开腹组(P<0.05)。结论急性发作期胆囊炎患者行腹腔镜胆囊切除术治疗的效果显著,创伤小,且可缓解患者的疼痛、改善患者的生存质量。

Objective To explore the clinical effect of laparoscopic cholecystectomy on acute episodecholecystitis.Methods A total of 80 patients with acute episodecholecystitis admitted in our hospital from January 2014 to December 2018 were selected. Among them, 39 patients underwent open cholecystectomy were set as open group and 41 patients underwent laparoscopic cholecystectomy were set as endoscopic group. The therapeutic effects of two groups were compared. Results The operation time, postoperative anal exhaust time, time to get out of bed and hospitalization time in the endoscopic group were significantly shorter than those in the open group, the amount of intraoperative blood loss was significantly less than that in the open group (P<0.05). The total incidence of complications in the endoscopic group was significantly lower than that in the open group (P<0.05). At 1 day after operation, PRI sensory score, PRI emotional score, VAS scores, PPI scores and total score of the two groups decreased, and those in the endoscopic groupwere lower than the open group (P<0.05). At 1 week after operation, the scores of subjective symptoms, psychological emotional, physiological function, social activity, digestive system specialty and total score of the two groups increased, and those in the endoscopic group were higher than the open group (P<0.05). Conclusion Laparoscopic cholecystectomy for acute episode cholecystitis has asignificant effect and less trauma, which can relieve pain and improve the quality of life of patients.
