摘要
目的分析超声颈后透明层厚度(NT)联合四维时空关联成像(STIC)对孕24~28周胎儿心脏畸形的诊断效能。方法回顾性分析5010例(5018胎)接受产前胎心筛查孕妇的临床资料,分析超声NT联合四维STIC对胎儿心脏畸形的诊断效能。结果5018例胎儿中443例超声NT值增厚(8.83%),四维STIC检出胎儿心脏畸形297例(5.92%),经引产尸解及产后随访证实47例胎儿心脏畸形(0.94%)。联合诊断准确度、特异度高于超声NT、四维STIC单独诊断(P<0.05);四维STIC诊断准确度、特异度高于超声NT(P<0.05);超声NT、四维STIC及联合诊断的灵敏度比较,差异不显著(P>0.05)。结论超声NT联合四维STIC在孕24~28周胎儿心脏畸形的诊断中具有更高价值。
Objective To analyze the diagnostic efficacy of ultrasound nuchal translucency thickness (NT) combined with four-dimensional spatio-temporal image correlation (STIC) in fetal heart malformation at 24 to 28 weeks of gestation. Methods The clinical data of 5 010 pregnant women (5 018 fetuses) undergoing prenatal fetal heart screening were retrospectively analyzed. The diagnostic efficacy of ultrasound NT combined with four-dimensional STIC for fetal heart malformation was analyzed. Results Of the 5 018 fetuses, 443 cases (8.83%) had thickened ultrasonic NT value, and 297 cases (5.92%) of fetal heart malformations were detected by four-dimensional STIC. After induction of delivery by autopsy and postpartum follow-up, 47 cases (0.94%) of fetal heart malformations were confirmed. The accuracy and specificity of combined diagnosis were higher than those of ultrasound NT and four-dimensional STIC alone (P<0.05). The diagnostic accuracy and specificity of four-dimensional STIC were higher than those of ultrasound NT (P<0.05). There were no significant differences in sensitivity of ultrasound NT, four-dimensional STIC and combined diagnosis (P>0.05). Conclusion Ultrasound NT combined with four-dimensional STIC has higher value in the diagnosis of fetal heart malformation at 24 to 28 weeks of gestation.
