左半与右半结肠癌K-RAS基因及肿瘤浸润T细胞表达的比较

摘要 目的对左半与右半结肠癌K-RAS基因及肿瘤浸润T细胞(TIL)表达进行对比分析。方法收集2017年1月至2019年1月手术治疗的结直肠癌患者315例。采集患者的临床资料,包括性别、年龄、淋巴转移情况、肿瘤分化程度及病理分期。采用二代测序法检测KRAS基因突变情况,免疫组化染色检测肿瘤组织中TIL(CD3^+、CD4^+T淋巴细胞)的表达情况,对左、右半结肠癌的临床特征及K-RAS基因突变、TIL表达情况进行比较。结果左、右半结肠癌患者的性别、年龄、淋巴转移状态、肿瘤分化程度及病理分期的比较差异均无统计学意义(均P>0.05);左半结肠癌K-RAS基因野生型的比例高于右半结肠癌,差异有统计学意义(χ^2=6.27,P<0.05);左半结肠癌中TIL的比例高于右半结肠癌,差异有统计学意义(P<0.05);右半结肠癌患者肝转移和肺转移比例均高于左半结肠癌,差异均有统计学意义(均P<0.05)。结论左半与右半结肠癌K-RAS基因及肿瘤浸润T细胞的表达存在差异。 Objective To compare the expression of K-RAS gene and tumor infiltrating T cells(TILs)in left and right colon cancer.Methods A total of 315 patients with colorectal cancer treated surgically in Huzhou Central Hospital from January 2017 to January 2019 were recruited.Clinical and pathological information including gender,age,lymphatic metastatic states,degree of differentiation and pathological stages were collected.The immunohistochemical staining was performed on the expression status of CD3^+and CD4^+T lymphocytes in colorectal cancer patients,and the next-generation gene sequencing was used to determined the mutation status of K-RAS gene.The expression of TILs and the mutation of K-RAS gene were compared between left and right colon cancer.SPSS software was used to analyze the relationship between the expression of TILs and K-RAS gene.Results There were no significant differences in gender,age,lymph node metastasis,differentiation degree or pathological stage between the left and right colorectal cancer patients(P>0.05).The proportion of wild-type K-RAS gene in left colorectal cancer was higher than that in right colorectal cancer(χ^2=6.27,P<0.05).The proportion of TILs in left colorectal cancer was higher than that in right colorectal cancer(P<0.05).The incidence of liver metastasis and lung metastasis in patients with right colon cancer was higher than that in patients with left colon cancer(P<0.05).Conclusion There are differences in K-RAS gene and T cell expression in left and right colon cancer.
