人参汤治疗心肾阳虚型不稳定性心绞痛患者的临床研究

Clinical Study of Renshen Decoction in the Treatment of Unstable Angina Pectoris of Deficiency of Heart-Yang and Kidney-Yang

目的:临床观察人参汤治疗心肾阳虚型不稳定性心绞痛患者的效果。方法:将符合纳入标准的60例不稳定性心绞痛患者随机分为两组,每组30例,两组均给予标准西医治疗,治疗组在标准西医治疗的基础上口服人参汤,疗程均为2周。比较两组治疗后的中医症候疗效、心电图变化、GRACE评分、血脂水平变化情况。结果:治疗组中医症候疗效总有效率86.7%(26/30)高于对照组60%(18/30)(P<0.05);治疗组心电图总有效率80%(24/30),对照组为53.3%(16/30),两组比较有统计学差异(P<0.05);治疗后GRACE积分组间比较有统计学差异(P<0.05)。两组治疗后TG、LDL-C水平均下降明显,组内及组间均有统计差异(均P<0.01)。两组TC水平下降显著(P<0.01);且治疗组优于对照组(P<0.05)。HDL-C水平治疗后两组组内及组间比较无统计学差异(P>0.05)。结论:在西医常规治疗基础上联合人参汤治疗心肾阳虚型不稳定性心绞痛,可进一步改善UA患者的中医证候、促进心电图缺血性改变的恢复、降低GRACE积分和血脂水平。

Objective: To observe the clinical effect of Renshen Decoction in treating unstable angina pectoris of deficiency of Heart-yang and Kidney-yang.Methods: 60 patients with unstable angina pectoris who met the inclusion criteria were randomly divided into the control group( n =30) and the treatment group( n =30).The control group was treated with standard western medication;on which basis,the treatment group was also treated with Renshen Decoction.The treatment course was two weeks.The curative effect of TCM symptoms,changes of electrocardiogram(ECG),GRACE scores and levels of blood lipid were compared between the two groups after the treatment.Results: The total effective rate of TCM symptoms was 86.7%(26/30) in the treatment group,which was significantly higher than 60.0%(18/30) in the control group( P <0.05).The total effective rate of ECG improvement was 80.0%(24/30) in the treatment group,which was obviously higher than 53.3%(16/30) in the control group( P <0.05).There was statistical difference in GRACE scores between the two groups after the treatment( P <0.05).After the treatment,the levels of TG and LDL-C were decreased significantly in the two groups,and there were statistical differences within and between the two groups( P <0.01).TC level was reduced greatly in the two groups( P <0.01),and the treatment group was better than the control group( P <0.05).There was no significant difference in HDL-C level within and between the two groups after the treatment( P >0.05).Conclusion: Western medicine combined with Renshen Decoction can further improve the TCM syndromes of unstable angina pectoris,it can promote the recovery of ischemic changes of ECG and reduce GRACE score and blood lipid level.
