抑郁症的证型与体质及其关系的文献研究

Literature Study on Syndrome and Constitution and Their Relationship of Depression

目的:为实现个体化诊疗提供依据,指导抑郁症的早期预防及临床选方用药。方法:计算机检索CNKI、VIP、WANFANG、CBM数据库的相关文献,提取有关信息,并采用Spss24分析抑郁症常见证型与体质及其关系。结果:纳入76篇文献,Spss24分析结果显示肝郁气滞证在抑郁症、卒中后抑郁症和女性抑郁症中占比较大,肝郁脾虚证在老年抑郁症和更年期抑郁症中占比较大,心肾不交证在血管性抑郁症占比较大。气郁质在抑郁症中居于首位。气虚质与心脾两虚证具有高相关性;气郁质与肝郁脾虚证、肝郁气滞证具有高相关性;阴虚质与心肾不交证具有高相关性;阳虚质与肝郁脾虚证、心脾两虚证和肝肾阴虚证具有高相关性;痰湿质与肝郁脾虚证具有高相关性;特禀质与血行郁滞证具有高相关性;平和质与肝郁气滞证、血行郁滞证、心肾不交证具有高相关性。结论:对于不同抑郁症患者,所采用的预防和治疗方式应各有侧重。医生应辨体、辨证相结合,更好地对抑郁症进行预防和治疗。

Objective:To provide the premise of individual diagnosis and treatment and guide the early prevention and clinical medicine of depression.Methods:Computer searched related published literatures from the CNKI,VIP,Wan Fang,CBM database and extracted relevant information,using Spss24 analysis the common constitution and syndromes and their relationship of depression.Results:Seventy-six literatures were included.Analysis showed:its proportion is relatively large that liver qi stagnation in depression,post-stroke depression and depression in women,liver depression and spleen deficiency syndrome in the elderly depression and menopausal depression,heart and kidney do not pay in vascular depression.Qi stagnation is the top priority in depression.Physique and syndrome with high correlation are qi deficiency and heart and spleen deficiency,qi stagnation and liver stagnation and spleen deficiency and liver qi stagnation,yin deficiency and heart and kidney stagnation,yang deficiency and liver stagnation and spleen deficiency and heart and spleen deficiency and liver-kidney yin deficiency,sputum dampness and liver stagnation and spleen deficiency,special sputum and blood stagnation syndrome,peacefulness and liver qi stagnation and blood stagnation and heart and kidney stagnation.Conclusion:For different types of depression,the methods of prevention and treatment should be different.Physicians diagnosis combined the constitution with syndrome can prevent and treat depression better.
