经椎板间隙入路脊柱内镜治疗游离型腰椎间盘突出症52例

52 Cases of Sequestered Lumbar Disc Herniation Treatment by Spinal Endoscopy Via Interlaminar Approach

目的:观察腰硬联合麻醉下行经椎板间隙入路脊柱内镜治疗游离型腰椎间盘突出症的临床疗效。方法:选取2017年1月-2018年1月本院收治的52例游离型腰椎间盘突出症患者,均行经椎板间隙入路手术治疗;记录术前及术后第1天和3、6个月腰腿VAS评分,采用改良Macnab疗效评分。结果:术后第1天和3、6个月腰腿VAS评分与术前比较有统计学意义(P<0.05),根据改良Macnab疗效评分标准,优良率为98.1%。结论:腰硬联合麻醉下行经椎板间隙入路脊柱内镜治疗游离型腰椎间盘突出症,近期临床疗效满意,且安全有效,值得临床推广。

Objective:To observe the clinical efficacy of spinal endoscopy(percutaneous transforminal endoscopic discectomy)for the treatment of sequestered lumbar disc herniation under combined spinal epidural anesthesia.Methods:From January 2017 to January 2018,52 patients with free style lumbar disc herniation were treated by translaminar intervertebral space approach,VAS scores of waist and leg were recorded before,and first day,3,6 months.The scores of VAS and 3,6 months after operation and with modified Macnab score.Results:The VAS score of waist and leg at the frist day,3 and 6 months after operation was significantly higher than that before operation.According to the improved Macnab score,the excellent and good rate was 96.2%.Conclusion:Spinal endoscopy under combined spinal-epidural anesthesia for treatment of sequestered lumbar disc herniation is safe and effective,which is worth popularizing in clinical.
