886例体检者幽门螺杆菌抗体分型检测结果分析 被引量:4

摘要 目的探讨幽门螺杆菌(HP)感染在健康体检者中的分布及流行状况。方法采用免疫印迹法同时检测血清HP四种抗体(CacA抗体、VacA抗体、UreA抗体、UreB抗体)水平,计算其阳性率,用统计学方法比较各组差异。结果886例健康体检者HP总感染率为63.9%(566/886),其中,I型322例,占总感染率的56.9%(322/566),Ⅱ型244例,占总感染率的43.1%(244/566).按照性别分组,男性共470例,HP阳性291例,感染率61.9%;女性共416例,HP阳性275例,感染率66.1%。不同性别组间感染率和分型进行x2检验,HP感染率、I型感染率和II型感染率差异均无统计学意义(P>0.05),按照年龄分布分为三组,<40岁组共144例,HP阳性76例,感染率52.8%;40-59岁组共525例,HP阳性361例,感染率68.8%;≥60岁组共217例,HP阳性129例,感染率59.4%。结果表明,随着年龄的增大,HP感染率先增后减,其中各个年龄段的I型感染率均比Ⅱ型感染率高。对不同年龄组进行x2检验,<40岁组与360岁组的HP阳性率及I型感染率与40-59岁组间差异均有统计学意义(P<0.05),而Ⅱ型感染率差异无统计学意义(P>0.05).结论体检人群HP的I型感染率在40~59岁组达最高,且免疫分型在体检人群中存在特殊的分布特征。 Objective To investigate the distribution and prevalence of Helicobacter pylori (HP) infection in healthy people by physical examination. Methods The levels of four antibodies (CacA antibody, VacA antibody, UreA antibody, UreB antibody) of HP were detected by Western blotting and the positive rate was calculated. Results The total infection rate of HP was 63.9%(566/886) in 886 healthy persons, of which 322 cases were type I , accounting for 56.9%(322/566) of the total infection rate, and 244 cases of type Ⅱ, accounting for 43.1%(244/566) of the total infection rate. According to the sex group, there were 470 males and 291 HP positive cases, the infection rate was 61.9%(291/470), the positive rate of HP was 66.1%(275/416) in women and 416 cases in females, x 2 test showed that there was no significant difference in HP infection rate, type I infection rate and type II infection rate among different sex groups (P> 0.05). According to the age distribution, 886 cases were divided into three groups. 144 cases were under 40 years old, the positive rate of HP was 52.8%(76/144). There were 525 cases in 40-59 age group and 361 cases in HP positive group, the infection rate was 68.8%(361/525). 217 cases were over 60 years old, 129 cases were HP positive, the infection rate was 59.4%(129/217). The results showed that HP infection increased first and then decreased with age, and the infection rate of type I was higher than that of type U in all age groups, x2 test showed that there were significant differences in HP positive rate and type I infection rate between the group of under 40 and over 60 years old and the group of 40-59 years old (P < 0.05), but there was no significant difference in type U infection rate (P > 0.05). Conclusion The infection rate of HP type I was the highest in the 40-59 age group, and the immunophenotype had special distribution characteristics in the physical population.
