宫腹腔镜联合治疗剖宫产术后瘢痕妊娠的临床研究

Clinical Study on Hysteroscopy Combined with Laparoscopy in the Treatment of Scar Pregnancy After Cesarean Section

目的探讨宫腹腔镜联合治疗剖宫产术后瘢痕妊娠的临床价值。方法纳入我院2016年1月至2018年10月收治的84例剖宫产术后瘢痕妊娠患者作为研究对象,随机分为两组各42例。研究组患者联合应用宫腹腔镜切除瘢痕妊娠病灶,对照组患者单纯应用宫腔镜切除瘢痕妊娠病灶,比较两组患者的手术相关指标及术后恢复情况。结果研究组的手术时间、术中出血量及住院天数均显著多于对照组(P <0.05)。研究组的术后阴道流血时间、β-HCG恢复正常时间及月经复潮时间均显著短于对照组(P <0.05)。结论宫腹腔镜联合治疗剖宫产术后瘢痕妊娠的临床疗效确切,在切除妊娠病灶的同时可修补子宫下段缺损。

Objective To investigate the clinical value of hysteroscopy combined with laparoscopy in the treatment of scar pregnancy after cesarean section. Methods 84 cases of patients with scar pregnancy after cesarean section admitted to our hospital from January 2016 to October 2018 were enrolled as research objects, and randomly divided into two groups, with 42 cases in each group. The study group received resection of scar pregnancy lesions through hysteroscopy combined with laparoscopy, while the control group received resection of scar pregnancy lesions through hysteroscopy. The surgery related indicators and postoperative recovery were compared between the two groups. Results The operation time, intraoperative blood loss and hospitalization time of the study group were significantly higher than those of the control group(P <0.05). The postoperative vaginal bleeding time, β-HCG return to normal time and menstrual rehydration time of the study group were significantly shorter than those of the control group(P <0.05). Conclusions Hysteroscopy combined with laparoscopy in the treatment of scar pregnancy after cesarean section has exact clinical curative effect, which can repair the defect of the lower uterus while excising the pregnancy lesion.
