435例结直肠癌患者术前血清前白蛋白水平与肿瘤的临床病理特征的关系研究

Relationship between serum prealbumin level before surgery and clinicopathological features of colorectal cancer: A analysis of 435 patients

目的探讨结直肠癌患者术前血清前白蛋白与肿瘤的临床病理特征的关系。方法回顾性分析2013年6月至2015年11月在广西医科大学第一附属医院结直肠肛门外科行结直肠癌根治术的435例患者临床资料,探讨在结直肠癌患者术前血清前白蛋白水平与肿瘤的临床病理特征的关系。结果男性术前血清前白蛋白表达水平低于女性,高、中分化腺癌患者表达水平低于低分化腺癌患者,T1~2期患者表达水平低于T3~4期患者,N0患者表达水平低于N1~2期患者,M0期患者表达水平低于M1期患者,TNMⅠ~Ⅱ期患者表达水平低于Ⅲ~Ⅳ期患者,DukesA^B期患者表达水平低于C^D期患者,CA199、CA125、CA242、CA724水平等肿瘤标志物阴性的患者表达水平低于阳性患者,差异均有统计学意义(均P<0.05)。术前血清前白蛋白水平与性别、M分期、TNM分期、Dukes分期、CA199、CA242、CA724水平有关,差异均有统计学意义(均P<0.05)。结论结直肠癌患者术前血清前白蛋白水平与性别、M分期、肿瘤TNM分期、Dukes分期、CA199、CA242、CA724水平有关,其临床价值有待进一步研究。

Objectives To investigate the relationship between preoperative serum prealbumin level and clinicopathological features in patients with colorectal cancer. Methods This was a retrospective analysis of clinical data of 435 patients with colorectal cancer treated at Department of Colorectal & Anal Surgery, The First Affiliated Hospital of Guangxi Medical University, between June 2013 and November 2015. We investigate the relationship between preoperative serum prealbumin level and patients’ clinicopathological features. Results Preoperative serum prealbumin level was significantly higher in men than in women (P < 0.05). It was significantly lower in patients with highly to moderately differentiated adenocarcinoma than in patients with poorly differentiated adenocarcinoma, in patients of T1~2 than those of T3~4, in patients of stage N0 than those of stage N1~2, in patients of stage M0 than those of stage M1, in patients of stage TNM Ⅰ~Ⅱ than those of stage TNM Ⅲ~Ⅳ, in patients of stage Dukes A^B than those of stage C^D, and in patients with negative CA199, CA125, CA242, and CA724 than their counterparts, respectively (P < 0.05). There were significant correlation between prealbumin level and sex, M staging, TMN staging, Dukes staging, CA199, CA242, and CA724 (P < 0.05). Conclusion Preoperative serum prealbumin level was associated with sex, M staging, TNM staging, Dukes staging, CA199, CA242, and CA724 levels. Its value in clinical practice requires further studies.
