经鼻置入小肠减压管用于结直肠癌术后早期炎性肠梗阻治疗的临床效果观察

Effect of transnasal intestinal decompression tube in treating early inflammatory small bowel obstruction after colorectal cancer surgery

目的探讨经鼻置入小肠减压管用于结直肠癌术后早期炎性肠梗阻的疗效。方法回顾性分析南京市中医院2016年11月至2018年6月期间收治的4例采用经鼻置入小肠减压管治疗的结直肠癌术后早期炎性肠梗阻患者的临床资料。结果3例患者单纯采用经鼻置入小肠减压管等保守治疗,经鼻置入小肠减压管留置时间为7~19d;住院天数为35~43d;放置经鼻置入小肠减压管的手术时间为80~140min;经鼻置入小肠减压管后24h减压量为640~1400mL。1例先后采用手术及经鼻置入小肠减压管治疗的患者痊愈时间为41d,住院天数为82d。结论经鼻置入小肠减压管在结直肠癌术后早期炎性肠梗阻中的应用是安全有效的,本研究但样本量较小,更有说服力的结论需要更多的病例数和更长时间的随访研究来证明。

Objectives To investigate the effect of transnasal intestinal decompression tube in treating early inflammatory small bowel obstruction after colorectal cancer surgery. Methods We analyzed clinical data of four patients with early inflammatory small bowel obstruction after colorectal cancer surgery treated with transnasal intestinal decompression tube in Nanjing Hospital of traditional Chinese Medicine between November 2016 and June 2018. Results Three patients received transnasal intestinal de. compression tube along with other conservative treatment. Tube dwell time was 7 to 19 days. Duration of hospitalization ranged from 35 to 43 days. Duration of procedure for transnasal intestinal decompression tube ranged from 80 to 140 minutes. Amount of decompression 24 hours after the procedure ranged from 640 to 1 400 mL. One patient received both surgery and transnasal intestinal decompression tube. Time to healing for this patient was 41 days and duration of hospitalization 82 days. Conclusion Transnasal intestinal decompression tube is safe and effective in treating early inflammatory small bowel obstruction after colorec. tal cancer surgery, however, our analysis was based on small sample and our findings required larger sample with longer followup to confirm.
