老年人主观记忆抱怨与客观认知损害的关联性

Association of Subjective Memory Complaints with Objective Cognitive Impairments in the Elderly

目的:考查老年人主观记忆抱怨与客观认知损害的关系。方法:从社区募集1500名50岁以上的社区老年人,1302名老年人(男592人、女710人,平均年龄66.54±6.97岁,平均教育年限9.25±2.82年)完成简易老年人认知筛查问卷、临床访谈与检查及核心神经认知测验。结果:按主观记忆抱怨筛查标准,1302名社区老年人中,有629人(48.3%)存在主观记忆抱怨,其中461人(35.4%)为轻度主观记忆抱怨,168人(12.9%)为明显主观记忆抱怨;主观记忆抱怨组神经认知测验得分低于无主观记忆抱怨组(F>9.25, P<0.01),主观记忆抱怨者半数被核心神经认知测验诊断为轻中度认知损害(32.1%)或严重认知损害(18.1%),多数被临床诊断为MCI(56.3%)或老年痴呆(13.5%)。结论:主观记忆抱怨与客观认知损害密切相关,半数以上能被诊断为MCI或痴呆;主观记忆抱怨可作为AD早期诊断的重要线索和便捷指标。

Objective: To investigate the relationship between subjective memory complaints and objective cognitive impairments in the community elderly. Methods: 1500 elderly persons aged 50 years or older were recruited from the community, 1302 elderly persons completed Brief Elderly Cognitive Screening Inventory, clinical interview and examination, and Core Neuro-cognitive Test. Results: According to the subjective memory complaint screening standard, 629(48.3%) had subjective memory complaints(SMC), out of them, 461(35.4%) persons with mild SMC, and 168(12.9%) persons with obvious SMC;The SMC group scored lower than the non SMC group(F>9.25, P<0.01) on the Core Neuro-cognitive Test, and half SMCs were diagnosed with mild or moderate cognitive impairment(32.1%) or severe cognitive impairment(18.1%) on the Core Neuro-cognitive Test, most SMCs were clinically diagnosed as MCI(56.3%) or dementia(13.5%). Conclusion:SMC is closely related to objective cognitive impairment, and most SNCs can be diagnosed as MCI or dementia;SMC can be used as an important clue and convenient index for early diagnosis of AD.
