高危型人乳头瘤病毒感染与中医体质的相关性研究

Correlation Research between High-Risk Human Papillomavirus Infection and TCM Constitution

目的:对高危型人乳头瘤病毒(high-risk human papillomavirus,HR-HPV)感染患者进行中医体质辨识,探讨中医体质与HR-HPV感染的关系,为调理体质防治HR-HPV感染提供依据。方法:收集HR-HPV感染患者60例(感染组),健康女性60例(对照组),按《中医体质分类与判定》标准分别对2组患者进行中医体质调查,调查结果用SPSS19. 0进行统计分析。结果:1) 2组患者中医体质特征分布调查结果显示感染组患者单一体质18例(30. 0%),其中单一平和质5例(8. 3%),单一偏颇体质13例(21. 7%);兼杂体质42例(70. 0%),其中平和兼杂体质17例(28. 3%),偏颇兼杂体质25例(41. 7%)。对照组:单一体质44例(73. 3%),其中单一平和质39例(65. 0%),单一偏颇体质5例(8. 3%);兼杂体质16例(26. 7%),其中平和兼杂体质6例(10. 0%),偏颇兼杂体质10例(16. 7%)。二组比较,单一体质和兼杂体质体质分布、平和质和偏颇体质体质分布均有显著差异(均P <0. 01)。2) 2组患者中医体质类型分布调查结果显示感染组体质分布:湿热质、平和质>阳虚质>气虚质>血瘀质>痰湿质、气郁质>阴虚质>特禀质;对照组:平和质>气郁质>湿热质、阳虚质>气虚质>痰湿质>阴虚质>血瘀质、特禀质。2组比较,感染组平和质出现的频次明显降低,常见的偏颇体质以湿热质、阳虚质和气虚质多见。3)中医体质与HR-HPV感染的相关性分析:2组的9种体质进行单因素分析,平和质、气虚质、阳虚质、阴虚质、湿热质和血瘀质差异有统计学意义(P <0. 05),将上述体质作为自变量、HR-HPV感染作为因变量进行二分类Logistic回归分析发现,湿热质是HR-HPV感染的危险因素(OR:3. 087,95%CI:1. 027~9. 283)。结论:HR-HPV感染患者与健康女性体质分布存在差异,湿热质是其危险因素,调整患者体质的偏颇,对易感体质进行早期干预可预防HR-HPV感染的发生。

Objective:To identify the TCM constitution of HR-HPV infection,and to explore the relationship between the TCM constitution and the HR-HPV infection,then to provide evidence for regulating physical fitness to prevent HR-HPV infection.Methods:60 cases of HR-HPV infection( the infection group) and 60 cases of healthy women( the control group) were collected.According to the Category and Judgement of TCM Constitution,two groups were investigated on TCM Constitution,and the results were statistically analyzed by SPSS19.0.Results:1) the comparison of the TCM constitutional characteristics of the two groups showed that the infection group had 18 cases( 30.0%) of the single constitution,including 5 cases( 8.3%) of single harmonious constitution,and 13 cases( 21.7%) with single biased constitution;42 cases( 70.0%) of the mixed constitution,including 17 cases( 28.3%) of harmonious and mixed constitution,and 25 cases( 41.7%) with biased and mixed constitution.The control group had 44 cases( 73.3%) of the single constitution,including 39 cases( 65.0%) of single harmonious constitution,and 5 cases( 8.3%) with single biased constitution;16 cases( 26.7%) of the mixed constitution,including 6 cases( 10.0%) of harmonious and mixed constitution,and 10 cases( 16.7%) with biased and mixed constitution.There were significant differences in the distribution of the single constitution and the mixed constitution of the two groups( P < 0.01);There was also a significant difference in the distribution of the harmonious constitution and the biased constitution of the two groups( P < 0.01).2) the comparison of the TCM constitutional type of the two groups showed that the order of the infection group according to the frequency of high and low order was the damp-heat constitution and the harmonious constitution,the yang-deficiency constitution,the qi-deficiency constitution,the blood-stasis constitution,the phlegm-dampness constitution and the qi-stagnation constitution,the yin-deficiency constitution,the special constitution;the order of the control group was the harmonious constitution,the qi-stagnation constitution,the damp-heat constitution and the yang-deficiency constitution,the qi-deficiency constitution,the phlegm-dampness constitution,the yin-deficiency constitution,the blood-stasis constitution and the special constitution.The frequency of the occurrence of the harmonious constitution in the infection group were decreased obviously,and the common biased constitution were mostly seen in the damp-heat constitution,the yang-deficiency constitution and the qi-deficiency constitution.3) the correlation analysis of the TCM constitution and the HR-HPV infection:the TCM constitutional type of the two groups were compared by single factor,the difference in the harmonious constitution,the qi-deficiency constitution,the yang-deficiency constitution,the yin-deficiency constitution,the damp-heat constitution and the blood-stasis constitution were statistically significant( P <0.05).Taking the above constitution as the independent variable and the HR-HPV infection as the dependent variable,two classification Logistic regression analysis showed that the damp-heat constitution was a risk factor of the HR-HPV infection( OR:3.087,95% CI:1.027 ~ 9.283).Conclusion:There were differences in the constitutional distribution of the healthy women and the HR-HPV infected patients.The damp-heat constitution was a risk factor of the HR-HPV infection.To adjust the bias of the patients’ constitution,and to intervene early in the susceptible constitution could prevent the occurrence of the HR-HPV infection.
