摘要
目的:探讨核磁共振成像(MRI)测量表观扩散系数(ADC)、信号强度比值(SIR)与骨质疏松腰椎疾病发生发展的相关性。方法:以2011年3月~2017年1月行腰椎骨密度检查的235例患者为研究对象,经双能X线吸收法(DXA)检查分为骨量减少组(52例)、骨质疏松组(85例)和骨量正常组(98例),所有受试者通过MRI扫描进行化学位移成像,测量L2~4椎体的ADC值和SIR值,分析ADC、SIR与DXA所测骨密度(BMD)间的相关性。结果:骨质疏松组ADC值显著低于骨量减少组和骨量正常组,SIR值显著高于骨量减少组和骨量正常组,差异有统计学意义(P<0.05);ADC值与BMD呈正相关(r=0.325, P=0.006),SIR值与BMD呈负相关(r=-0.601, P=0.001)。结论:通过MRI测量的ADC、SIR与BMD值相关性良好,二者能够对骨质疏松的椎体进行定量评价,很好地反映骨质疏松腰椎疾病的发生发展情况。
Objective To investigate the correlation between the apparent diffusion coefficient(ADC) and signal intensity ratio(SIR) measured by magnetic resonance imaging(MRI) and the occurrence and development of osteoporotic lumbar vertebrae.Methods A total of 235 patients receiving bone mineral density(BMD) scans from March 2011 to January 2017 were enrolled,and then divided into osteopenia group(n=52), osteoporosis group(n=85) and normal group(n=98) according to the results of dual-energy X-ray absorptiometry. The ADC and SIR of the L2-4 vertebral body of all subjects were measured by MRI, and the correlation between the MRI measurements of ADC and SIR and the BMD measured by dual-energy X-ray absorptiometry were analyzed. Results Compared with those of osteopenia group and normal group, the ADC of osteoporosis group was significantly decreased, while the SIR was significantly increased, with statistical significance(P<0.05). ADC was positively correlated with BMD(r=0.325, P=0.006), and SIR was negatively correlated with BMD(r=-0.601, P=0.001). Conclusion The ADC and SIR measured by MRI have good correlations with BMD. Both ADC and SIR can be used to quantitatively evaluate osteoporotic vertebral body and reflect the occurrence and development of osteoporotic lumbar vertebrae.
